摘 要:目的探讨胸部X线检查联合CT在早期肺部感染中的诊断效果,为早期肺部感染的正确诊断提供依据。方法选取医院2010年12月-2012年12月收治的60例早期肺部感染患者进行研究,分别进行胸部X线联合CT检查,并与单纯的X线相比观察肺部感染诊断情况;另选择同期60名健康体检者作为对照组,观察两组胸部X线及CT检测征象,并进行对比分析,采用SPSS13.0进行统计处理。结果 60例患者经X线联合CT检查,确诊肺部感染55例,诊断率达91.67%,其中4例经胸部X线及痰液检查确定非肺部感染,诊断阳性率为85.00%,显著高于单纯X线片检查,差异有统计学意义(P<0.05);与健康受试者对照发现,肺部感染的患者X线和CT检查可见肺部阴影密度、形态、范围、数量以及阴影所在部位与周围正常肺组织的关系显著不同。结论 X线因其低辐射、价廉、普及率高成为肺部感染初步筛查及治疗后随访的首选方法,而CT检查的敏感性及特异性明显优于X线,对于临床高度怀疑肺部感染而胸片正常者,可以选择CT扫描,两者联合显著提高了早期肺部感染的诊断率。OBJECTIVE To study the effect of chest X-ray and CT examination in the diagnosis of lung infections so as to provide reference for correct diagnosis for early lung infections.METHODS Totally 60 patients with early lung infections from Dec.2010 to Dec.2012 in our hospital were studied.All patients were given with X-ray and CT checks which were compared with single X-ray check for observation of lung infections.Another 60 healthy volunteers were selected as the control group.The two groups were observed for chest X-ray and CT detecting signs and the comparative analysis was conducted.RESULTS Totally 55 in 60patients were confirmed to have lung infections by X-ray and CT examination,with the diagnosis rate of 91.67%,of which 4cases were determined to be non-lung infections by chest X-ray and sputum examination,indicating the positive rate for diagnosis was 85.00%,significantly higher than that of simple X-ray examination(P0.05).Compared with healthy subjects,Xray and CT examinations showed significant differences in correlation of lung shadow density,shape,scope,quantity and shadow sites with the surrounding normal lung tissues.CONCLUSION Because of its low radiation,low cost,and high penetration,X-ray has become a preliminary screening and treatment method for lung infections,while the sensitivity and specificity of CT is superior to X-ray.For patients highly suspected to have lung infections whose chest X-ray shows normal,CT scans could be chosen,and its combination with X-ray check significantly improves the diagnosis rate of early lung infections.
