良性阵发性位置性眩晕患者复发后首诊科室分析

摘　　要：目的:探讨良性阵发性位置性眩晕(BPPV)患者首次复发首选就诊科室的影响因素。方法:回顾性分析2012年1月至2013年12月确诊为BPPV的101例首次复发后复诊患者的临床资料。结果:首次复发首诊耳鼻喉科66例(65.35%),首诊非耳鼻喉科者35例(33.02%),复发首诊耳鼻喉科和非耳鼻喉科的男、女及不同年龄组间无明显差异(P>0.05);伴随耳科疾病、不伴随全身疾病及功能检查正常者复发首诊耳鼻喉科的人数多(P均<0.001)。对BPPV的认知程度4-7分组、8-10分组首诊耳鼻喉科的人数明显高于0-3分组(P均<0.05),但4-7分组和8-10分组间首诊耳鼻喉科的人数无明显差异(P>0.05)。结论:复发后复诊的BPPV患者就诊时选择科室的差异与患者的性别和年龄无关,与患者对BPPV认识程度以及其伴随疾病有关,也与临床辅助功能检查结果的解读有关。Objective： To explore the influence factors of first visiting department of patients with first recurring benign paroxysmal positional vertigo （BPPV）. Methods： The clinical data of 101 patients with first recurring BPPV were analyzed retrospectively from Jan 2012 to Dee 2013. Results： The first recurrence cases of otolaryngology were 66 （65.35%） and those of non-otolaryngology were 35 （33.02%）. There was no significant correlation between of recurrence of primary otolaryngology and non otolaryngology differences between men and women, and the age difference had no significant correlation （both P 〉 0.05 ）. Most the patients with otology diseases or systemic diseases or normal function chosed otolaryngology （all P 〈 0.001 ）. The first option otolaryngology in patients with BPPV cognitive degree in 4-7 score group and 8-10 score group were significantly higher than that in 0-3 score group, but there was no significant difference between 8-10 score group and 4-7 score group. Conclusion： The difference of first visiting department of patients with first recurring shows no correlation with the gender and age, but correlate with their knowledge and awareness of BPPV and concomitant diseases, as well as the interpretation of clinical auxiliary function test results.
