摘　　要：目的 比较不同植皮方式在重度关节烧伤患者中的修复效果及对微循环的影响.方法 选取2011年9月～2013年3月收治的96例重度关节烧伤患者为研究对象.将其随机分为A组（切痂植皮组）48例和B组（削痂植皮组）48例,然后对两组患者不同关节的治疗总有效率及不同时间的局部微循环指标进行比较.结果 B组不同关节的治疗总有效率均高于A组,治疗后2、5及10d的局部红细胞聚集情况及血管清晰程度、血流灌注量则均好于A组,A组治疗后5及10d均好于治疗后2天,差异均有显著性（均P＜0.05）.结论 削痂植皮可有效提高重度关节烧伤患者治疗的总有效率,并对局部微循环发挥着积极的改善作用.Objective To compare the application effect of different skin grafting methods in patients with severe joint burn and influence for the microcirculation.Methods 96 patients with severe joint burn were were randomly divided into group A（escharectomy and skin grafting group,48 cases）and group B（eschar shaving and skin grafting group,48 cases）.The total effective rates of different joints and local microcirculation indexes at different times in two groups were compared.Results The total effective rates of group B with different joints were all higher than those of group A.The local erythrocyte aggregation state,vascular clarity degree and blood perfusion at 2nd,5th and 10th day after the treatment were all better than those of group B.Those indexes of group A at 5th and 10th day after treatment were all better than those at 2nd day after the treatment （P〈0.05）.Conclusion Treatment of severe joint burn patients with tangential excision has a significant effect and it also plays improvement role for the local microcirculation of patients.
