2009―2012年本院临床药师参与临床会诊的回顾性分析

摘　　要：目的分析近4年本院临床药师参与的临床会诊工作和效果,为临床药师参与临床实践的模式、方向提供参考。方法调取本院2009―2012年临床药师参与会诊的病例,从会诊科室分布、会诊数量、会诊目的、临床采纳程度以及会诊的临床效果进行回顾性分析。结果 2009年本院临床药师会诊5个科室(10例会诊),2010年8个科室(23例会诊),2011年13个科室(64例会诊),2012年26个科室(139例会诊);临床科室请临床药师会诊目的主要以抗菌药物的选择、调整和停药居多,占73.3%;临床药师会诊意见总体采纳225例(95.3%),未采纳的11例(4.7%);患者会诊后好转209例(88.6%),疗效不明显或恶化的9例(3.8%),因其他原因放弃治疗或死亡的7例(3.0%)。结论临床药师参与临床会诊对临床合理用药提供了一定的帮助,但目前仍存在一些不足,临床药师需要不断学习临床和药学知识,才能提高会诊质量。AIM To explore the role of clinical pharmacist participating in clinical consultation, providing a feasible practice mode or path for clinical pharmacists to take part in the therapeutic process. METHODS By obtaining the consultation cases in our hospital from 2009 to 2012, a retrospective analysis consist of the quantity, the distribution of departments, the purpose, the acceptations and the clinical outcome of consultation cases. RESULTS In 2009, the total number of clinical departments applying for consultation was 5 （l0 cases）, 8 （23 cases） in 2010, 13 （64 cases） in 2011, and 26 （139 cases） in 2012. The consultation were mainly involved in the selection, adjustment and withdrawal of antibacterial agents, account for 73.3%. The suggestion of pharmacists in 255 cases （95.3%） had been accepted, 11 cases were unadopted （4.7%）, and 209 cases with a fine outcome （88.6%）, 9 cases without obvious improvement, even with a deterioration （3.8%）, 7 cases had quit treating or died for other reasons （3.0%）. CONCLUSION The results indicate that clinical pharmacist participate in clinical consultations provide some help on rational drug use, but there are also some deficiencies. In order to improve the quality of the consultations, the clinical pharmacist must study the knowledge of clinical medicine and pharmacy.
