摘  要:目的通过自身对比双能量减影与数字减影图像质量,探讨第二代双源CT三期双能量扫描模式双期脑血管减影成像的临床初步应用。方法 20例三期双能量双源CT脑血管检查按减影法分成A组:双能量减影;B组:100kv Neuro DSA数字减影。计算2组图像信噪比(SNR)与对比噪声比(CNR)及减影时间,按去骨程度及血管显示与切割程度,采取双盲法分别对图像评分(1-5分)。比较2组间统计学差异。结果减影时间A组短于B组(2.07±0.19min VS2.27±0.13min,P<0.05)。A组动、静脉期SNR(29.89±12.20,30.49±7.34)均高于B组(24.42±9.99,27.17±6.59),差异有统计学意义(P<0.05)。2组动、静脉期CNR差异均不显著(P>0.05)。2组动、静脉期图像去骨评分差异均不显著(P>0.05),A组动脉期血管评分低于B组(3.95±0.97 vs 4.95±0.23,P<0.05),主要为部分颈内动脉管腔局部缺损。2组静脉期评分差异不显著。三期双能量扫描DLP平均为416.21±26.10 mGy·cm,较双期双能量扫描增加约10%。结论双源CT三期双能量扫描模式的双期脑血管成像,增加少许辐射剂量,可同时利用双能量与数字减影的优点来相互弥补诊断,推荐临床急诊脑血管检查。Objective To assess the initial clinic application of double phase subtraction angiography with second generation dual-source CT in 3 phases dual-energy scan mode, Self-contrast method was used to compared dual-energy subtraction with 100kv Neuro DSA subtraction. Methods 20 patients underwent 3 phases dual-energy scan mode CT angiography were divided into group A (with dual-energy subtraction) and group B(with 100KV Neuro DSA subtraction).The signal-to-noise ratio(SNR) and contrast-to- noise ratio(CNR) of double phrase subtraction images were measured,and the average postprocessing time of subtraction was calculated in two groups,respectively.The double phase subtraction images quality were graded by 2 independent readers on a 5-point scale (5=excellent) in double-blind, according to various degrees of the residual bones,as well as the vessel displayed and luminal integrity,respectively.Differences between two groups were analyzed.Results The average postprocessing time of subtraction in group A was shorter than in group B(2.07 ± 0.19min vs2.27 ± 0.13min,P 〈 0.05).The image SNR of Arterial and Venous phase in group A(29.89 ± 12.20,30.49 ± 7.34) were higher than in group B(24.42 ± 9.99, 27.17 ± 6.59),P 〈0.05,respectively.The double phrase image CNR showed no difference between 2 groups (P 〉 0.05). There were no singnificant difference in the bone removal image scores of double phase between 2 groups respectively (P 〉 0.05).The vessel image scores of Arterial phase were lower in group A than in group B(3.95 ± 0.97 vs 4.95 ± 0.23,P〈0.05).Some Internal carotid has not good luminal integrity in group A. The vessel image scores of Venous phase showed no difference between 2 groups (P 〉 0.05). The mean dose length product (DLP) of 3 phases scan mode was 416.21± 26.10 mGy · cm,an increase of 10% over the double phase scan mode.
