彩色多普勒超声对小乳腺癌与乳腺腺病的鉴别诊断

摘　　要：目的探讨多普勒彩超在区分诊断小乳腺癌与乳腺腺病中的临床应用效果。方法回顾性分析经过手术及病理确诊的小乳腺癌患者59例(小乳腺癌组)及乳腺腺病患者59例(乳腺腺病组)的临床资料,比较两组在2D声像图病灶形态特征、血流分级及血流动力学特征上的差异。结果小乳腺癌组患者的病灶形态不完整,边缘光滑程度差,回声多不均匀,后方回声衰减程度大,病灶钙化严重,纵横比明显偏高。小乳腺癌患者2D声像图病灶形态特征与乳腺腺病患者比较,SPVmax及RI较高,且血流分级以Ⅱ、Ⅲ级为主。两组检查结果差异有统计学意义(P<0.05)。结论多普勒超声检查可以有效地鉴别诊断小乳腺癌与乳腺腺病,区分其病灶形态及血流信号特征,值得临床推广。Objective To explore the diagnostic value of color Doppler ultrasound in small breast cancer and proliferation of mammary gland. Methods The clinical data of 59 patients with small breast cancer （the small breast cancer group） and 59 patients with proliferation of mammary gland （the prolifera- tion of mammary gland group） who were made definite diagnosis by surgery and pathology in our hospital were analyzed retrospectively. The 2D sonogram lesions morphological characteristics, grade of blood flow, hemodynamic characteristics between patients of the two groups were compared. Results There were incomplete lesion morphology, less smoother edge, uneven echo, posterior echo attenuation, severe calcification, higher aspect ratio in the small breast cancer group. Compared with the 2D sonogram lesions morphological characteristics of the proliferation of mammary gland group, the values of SPVmax and RI in the small breast cancer group were higher, and patients with blood flow at II and III grades was predominant. There were significant difference between the two groups （ P 〈 0.05 ）. Conclusion Color Doppler ultrasound can make differential diagnosis of breast lesions effectively according to lesions characteristics and blood flow signal characteristics, which has high value of clinical application.
