远红外穴位敷贴与药灸联合西药治疗稳定期慢性阻塞性肺病随机平行对照研究

摘　　要：[目的]观察远红外穴位敷贴与药灸联合西药治疗稳定期慢性阻塞性肺病疗效。[方法]使用随机平行对照方法,将80例门诊患者按随机数字表分为两组。对照组35例支持、对症治疗。治疗组34例1远红外穴位敷贴,主穴:选肺俞、定喘、膻中、大椎,临床辨证加减风门、丰隆、膏肓等,1次/d;2药灸,主穴:大椎、肺俞、定喘,根据临床辨证加减隔俞、风门等,使用白芥子、延胡、甘遂、细辛研成细粉,加入姜汁,做成直径5cm药饼,用艾柱灸,3次/周;西药治疗同对照组。连续治疗2周为1疗程。观测临床症状、肺功能、MRC评分、不良反应。治疗1疗程,判定疗效。观察期间治疗组6例,对照组5例,失联中断治疗。[结果]肺功能两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。MRC评分两组均有明显降低(P<0.05),治疗组降低优于对照组(P<0.01)。[结论]远红外穴位敷贴与药灸联合西药治疗稳定期慢性阻塞性肺病,疗效满意,无副作用,值得推广。[ Objective ] To study effect of far infrared acupoint and moxibustion combined with western medicine in treatment of stable chronic obstructive pulmonary disease. [ Methods ] By using a randomized controlled method, 80 patients were randomly divided into two groups. During the period of observation 6 cases in the treatment group and 5 cases in the control group were lost and interrupt treatment. Control support and symptomatic treatment were administrated in 35 cases of control group. The treatment group of 34 cases were administrated with （1） far infrared acupoint application, the main points are ： Dingchuan, Feishu Shanzhong, Dazhui;the supplement points are Fenglong, Gaohuang according to clinical syndrome differentiation and damper, 1 times /d; （2） moxibustion, main points：Dazhui, Feishu, Dingchuan, according to clinical syndrome differentiation and Geshu, Fengmeng. By grinding Baijiezi, Yanhu, Gansui and Xixing into a fine powder, ginger juice were added to make a diameter of 5cm medicine cake and to Moxa Moxibustion, 3 times / week;western medicine treatment were the same to the control group. Continuous treatment for 2 weeks was 1 course of treatment. Observation of clinical symptoms, pulmonary function, MRC score, adverse reaction during 1 courses of treatment and then the curative effect was evaluated. [ Results ] The pulmonary function of two groups were improved （ P〈 0.05, P〈0.01 ）, the treatment group was better than the control group （ P 〈 0.05, P〈0.01 ） . The MRC score of two groups were significantly decreased （P〈0.05）, the treatment group decreased than that in the control group （ P〈 0.01 ） . [ Conclusions ] Moxibustion, far infrared acupoint combined with western medicine in the treatment of stable chronic obstructive pulmonary disease had good curative effect and no side effect was observed, which is worth of promoting.
