2011年-2013年北京市西城区儿童A组溶血性链球菌的emm基因分型

摘　　要：目的了解2011年-2013年北京市西城区儿童携带A组溶血性链球菌(GAS)的M蛋白基因(emm)各基因型别分布情况。方法于2011年-2013年每年5月-7月,从北京市西城区辖区内两家医院采集猩红热或咽峡炎及扁桃体炎儿童患者的咽拭子,培养分离获得121株GAS,提取DNA,应用PCR扩增emm基因,进行测序,确定GAS的emm基因型别,并比较不同年度、不同人群、不同诊断病例来源标本基因型别的差异。结果 3年间121份GAS标本中共检测出了3个基因型分别为emm1、emm12、emm75,emm1为24株,emm12为96株,emm75为1株。检出了10个基因亚型,分别为emm12.0、emm12.1、emm12.19、emm12.29、emm12.30、emm12.38、emm1.0、emm1.33、emm1.76、emm75.0。结论2011年-2013年间北京市西城区儿童携带GAS的emm基因型别中,主要流行的基因型是emm12和emm1,主要流行的基因亚型是emm12.0和emm1.0,且emm1.0有逐年增多的趋势。emm基因型目前在北京西城区人群分布上在性别、年龄、病例诊断类型上无差异。仅在北京市和外省市人群中emm12和emm1的分布有差异(P<0.05)。Objective To investigate the distribution of each genotype of M protein （emm） gene in group A hemolytic streptococcus（GAS） isolated from children in Xicheng district of Beijing from 2011 to 2013. Methods From May to July of each year, the throat swabs were collected from children with scarlet fever or pharyngitis, or tonsillitis for culture and isolation of GAS. DNA was extracted from these GAS for PCR amplification, sequencing and genotyping of emm gene, then the genotypes differ- ence of emm gene were compared among specimens from different years, different people and different diagnosis source cases. Results Three genotypes were detected in 121 GAS isolates from 2011 to 2013, including 24 emm 1 strains, 96 emm 12 strians, 1 emm 75 strain. The 3 genotypes can be subdivided into 10 gene subtypes, emm 12. 0, emm 12. 1, emm 12. 19, emm 12. 29, emm 12. 30, emm 12. 38, emm 1. 0, emm 1.33, emm 1.76, emm 75. 0. Conclusion emm 12 and emm 1 were prevalent genotypes in GAS isolated from children in Xicheng district of Beijing from 2011 to 2013, while the main subtypes were emm 12. 0 and emm 1.0, and the emm 1.0 was increasing year by year. At present, the emm genotypes distribution showed no difference in population of Xicheng district of Beijing in gender, age and diagnosis types. But the difference was found in distribution of emm 12 and emm 1 between population in Beijing city and other cities（P 〈 0.05 ）.
