摘 要:目的分析天津地区尿路致病性大肠埃希菌(UPEC)的分子流行病学特点,比较1型菌毛黏附特性、遗传谱系分型与其耐药性的关系,为临床预防和治疗UPEC感染提供理论依据。方法通过3重2步PCR对菌株进行遗传谱系分型,酵母细胞黏附实验检测1型菌毛的黏附情况,并检测13种常用抗菌药物的药物敏感性,比较3者之间的关系。结果遗传谱系分型各组间的黏附特性进行两两比较:A组较B1组、B2组和D组黏附阳性率低(P<0.008)。黏附特性对耐药的影响:头孢吡肟耐药多见于黏附阳性组(χ2=6.38,P<0.05),头孢呋辛耐药多见于黏附阳性组(χ2=4.55,P<0.05)。遗传谱系分型与药敏之间的关系:对头孢哌酮/舒巴坦耐药株中,B2组少见(χ2=6.38,P<0.05);对头孢西丁耐药株中,D组多见(χ2=5.38,P<0.05)。结论以上3者之间的关系复杂,仍需完善调查分析UPEC的流行病学特点,从而指导临床合理用药、减少耐药菌的产生。Objective To investigate the molecular epidemiological characteristics of uropathogenic Escherichia coli(UPEC) in Tianjin, especially the relationship among type 1 fimbriae adhesion, phylogenetic groups and drug - resistance, so as to provide basis for clinical prevention and therapy of UPEC infection. Methods Phylogenetic groups of the UPEC strains was obtained by two - step triplex - PCR. Type 1 fimbriae adhesion was detected by yeast agglutination test. Drug susceptibility to 13 commonly used antibiotics was also detected to analyze the relationships among the three factors above. Results The pairwise comparison of type 1 fimbriae adhesion rates among phylogenetic groups showed the adhesion rate of group A was lower than group B1, B2 and D(P 〈0. 008). The relationship between type 1 fimbriae adhesion and drugresistance showed that cefepime resistance and cefuroxime resistance were more common in type 1 fimbriae adhesion positive strains (X2 = 6.38, P 〈 0.05 ; X2 = 4.55, P 〈 0.05). The relationship between phylogenetic groups and drug - susceptibility showed that there were less cefoperazone/snlbactam - resistant strains in group B2 (X2 = 6.38, P 〈 0.05 ), while more cefoxitin - resistant strains in group D (X2 = 5.38, P 〈 0.05). Conclusion The relationships among the three factors of UPEC are very complicated, further investigation and analysis on molecular epidemiologicat characteristics of UPEC is necessary to guide the clinical medication and reduce the drug - resistant strains.
