2型糖尿病患者合并骨质疏松症危险因素分析

摘　　要：目的探讨2型糖尿病(type 2 diabetes mellitus,T2DM)合并骨质疏松(osteoporosis,OP)症的临床特点及危险因素。方法共有T2DM患者200例,男性45例,女性155例,其中未合并OP患者107例,合并OP患者93例,比较2组一般临床资料及实验室数据的差异。并用二元logistic回归方法分析OP与相关危险因素的相关性。结果糖尿病合并OP组患者年龄较大(P<0.01),体重指数(body mass index,BMI)较小(P<0.05),心率较快(P<0.05)、骨钙素(osteocalcin,OC)高(P<0.01)、高密度脂蛋白(high density lipoprotein,HDL)高(P<0.01)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)低(P<0.01)、尿酸(uric acid,UA)低(P<0.01)、血尿素氮(blood urea nitrogen,BUN)低(P<0.05)、铁(ferrum,Fe)低(P<0.05)。二元logistic回归分析显示,年龄(OR=1.124,P=0.019)、BMI(OR=0.717,P=0.018)、OC(OR=1.189,P=0.009)、UA(OR=0.986,P=0.019)与OP有关。结论年龄、BMI、OC、UA与2型糖尿病合并骨质疏松症相关。Objective To investigate the risk factors of osteoporosis in type 2 diabetic patients.Methods Two hundred type 2 diabetic patients（45 males and 155 females） were enrolled in the study including 93 cased complicated with osteoporosis and 107 without osteoporosis.The clinical data and laboratory results were compared between two groups.The risk factors of osteoporosis were analyzed using logistic regression method.Results The older age（P 0.01）,lower body mass index（P 0.05）,higher heart rate（P 0.05）,higher osteocalcin（P 0.01）,higher HDL（P 0.01）,lower ALT（P 0.01）,lower Fe（P 0.05）,lower creatinine（P 0.01）,uric acid（UA,P 0.01） and BUN（P 0.05） were found in patients complicated with osteoporosis.Logistic regression analysis revealed that older age（OR = 1.124,P = 0.019）,lower BMI（OR = 0.717,P = 0.018）,higher osteocalcin（OR = 1.189,P = 0.009） and lower UA（OR = 0.986,P = 0.019） were correlated with osteoporosis.Conclusion The risk factors of osteoporosis in type 2 diabetic patients are age,BMI,osteocalcin and UA.
