摘 要:目的:检测正常糖耐量(NGT)人群及新诊断2型糖尿病(T2DM)患者血浆分泌型卷曲相关蛋白5(SFRP5)水平,探讨SFRP5与体脂、糖脂代谢、胰岛素抵抗等因素的关系。方法:招募58名NGT且体重正常人群及54例新诊断T2DM患者,依据2000年WHO推荐的亚洲成年人肥胖诊断标准将体重指数(BMI)<25kg/m2定义为正常体重。采用ELISA法测定血浆SFRP5含量,稳态模型评估胰岛素抵抗指数(HOMA-IR)。结果:①T2DM组血浆SFRP5水平明显低于NGT组[(9.74±3.25)比(13.56±3.49)μg/L,P<0.05];②相关分析显示血浆SFRP5水平与BMI、腰臀比(WHR)、腰围(WC)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、丙氨酸氨基转移酶(ALT)及γ-谷氨酰转移酶(γ-GT)均呈负相关(P<0.05);③多元逐步回归分析显示HOMA-IR、BMI是血浆SFRP5水平的独立影响因素(β1=-0.429、β2=-0.236,均P<0.05)。结论:SFRP5在T2DM患者中表达明显降低,可能参与了T2DM发病过程。Objective: To detect plasma secreted frizzled-related protein(SFRP) 5 level in normal glucose tolerance (NGT) and newly diagnosed type 2 diabetes mellitus(T2DM) subjects and to investi- gate the relationships between plasma SFRP5 level and body fat parameters, glucolipid metabo- lism and insulin resistance. Methods: Fifty-eight NGT subjects with normal weight and Fifty-four T2DM patients were enrolled. Normal weight was defined as body mass index (BMI)〈25 kg/m2 according to the World Health Organization -Western Pacfic Region diagnostic criteria (2000). Plasma SFRP5 was detected by ELISA and insulin resistance index (IR) was assessed by homeo- stasis model assessment (HOMA). Results: Plasma SFRP5 level in T2DM group was significant- ly lower than in NGT group ([9.74±3.25] vs [13.56±3.49] μg/L, P〈0.05); Correlation analysis showed that plasma SFRP5 level was negatively correlated with BMI, waist hip ratio (WHR), waist circumference (WC), fasting plasma glucose (FPG), glycosylated hemoglobin (HbAlc), fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA- IR), triglyceride (TG), alanine aminotransferase (ALT), and Gamma-glutamyl transferase (γ- GT) (P〈0.05). Multiple linear regression showed that HOMA-IR and BMI were independentrelated factors influencing the level of plasma SFRP5 (β1 = - 0. 429,β2 = - 0. 236, P〈0.05). Conclusion. Plasma SFRP5 level was decreased in T2DM subjects and it may contribute to the pathophysiology of T2DM.
