山东省40岁以上社区人群亚临床甲状腺功能减退症的流行病学研究

摘　　要：本研究通过对1967名40岁以上社区常住居民进行甲状腺功能检测,发现亚临床甲状腺功能减退（甲减）的患病率为7.4％,其中男性4.7％,女性9.1％,女性患病率显著高于男性（P＜0.05）.亚临床甲减组空腹胰岛素、稳态模型评估的胰岛素抵抗指数、血肌酐均高于甲状腺功能正常组（P＜0.05）.亚临床甲减组超重或肥胖患病率高于甲状腺功能正常组（P＜0.05）,两组代谢综合征患病率差异无统计学意义.Logistic回归分析发现性别（OR=1.981,P＜0.05）、HOMA-IR（OR=1.037,P＜0.05）、血肌酐（OR=1.013,P＜0.05）是亚临床甲减的独立相关因素.During February and March 2012,a cross-sectional survey of 1 967 adults aged 40 years or older from 2 urban communities （1 from Jinan and 1 from Jining） in Shandong Province was carried out.Thyroid functions were detected.The overall prevalence of subclinical hypothyroidism was 7.4% （4.7% in men and 9.1% in women）.The prevalence of subclinical hypothyroidism in female was higher than that in male （P〈0.05）.Fasting insulin,homeostasis model assessment insulin resistant index（HOMA-IR）,serum creatinine （SCr） in subclinical hypothyroidism group were higher than those in normal thyroid function group （All P 〈 0.05）.The prevalence of overweight or obesity in subclinical hypothyroidism group was higher than that in normal thyroid function group （P〈 0.05）,and the prevalence of MS between two groups showed no significant difference（P〉0.05）.Logistic regression analysis demonstrated that gender,HOMA-IR,and SCr（OR =1.981,1.037,1.013 respectively,all P〈0.05） were the relevant factors of subclinical hypothyroidism.
