摘  要:目的 探讨经皮穴位电刺激疗法和电针疗法对2型糖尿病患者血糖、糖化血红蛋白及体质指标的影响.方法 将60例2型糖尿病患者按随机数字表法分为电针组和Acu-TENS组,每组30例.2组患者在常规用药基础上,分别进行为期2个月,每日1次,每次30 min,每周5次的电针治疗和经皮穴位电刺激治疗.分别于治疗前、治疗2个月后(治疗后)及治疗结束后2个月(随访时)对2组患者进行餐后2h血糖、糖化血红蛋白、体质指数(BMI)、腰臀比、体脂百分比等指标检测,并进行统计学分析比较.结果 治疗后,电针组患者的餐后2h血糖(9.36±2.21)、糖化血红蛋白(6.76±1.01,mmol/L)、BMI(23.96±1.85,%)、腰臀比(0.88 ±0.01,%)、体脂百分比(26.68±3.16,%)等指标与Acu-TENS组的餐后2h血糖(8.99±1.82)、糖化血红蛋白(7.14±1.13)、BMI(23.73±1.28)、腰臀比(0.89±0.02,%)、体脂百分比(26.60±2.40,%)等指标均较治疗前有明显下降(P<0.05);随访时,除电针组患者糖化血红蛋白(7.46±1.71%)与治疗前及治疗后比较,差异均无统计学意义(P>0.05)外,电针组患者的餐后2h血糖(9.36±2.21,mmol/L)、BMI(25.96±1.98,%)、腰臀比(0.93±0.02,%)、体脂百分比(29.09±3.10,%)等指标与Acu-TENS组的餐后2h血糖(10.32±2.33,mmol/L)、糖化血红蛋白(7.87±1.72,%)、BMI(25.55±1.38,%)、腰臀比(0.93±0.01,%)、体脂百分比(28.78±2.48,%)等各项指标相比,虽有反弹,但是较治疗前仍有下降(P<0.05);2组患者各时间点的上述指标组间比较,差异均无统计学意义(P>0.05).结论 经皮穴位电刺激治疗在改善2型糖尿病患者血糖、糖化血红蛋白及体质指标方面与电针治疗相比,具有相同甚至略优于电针治疗的效果,值得推广应用.Objective To compare the effects of electrical stimulation applied transcutaneously to acupoints with electroacupuncture on blood glucose,glycosylated hemoglobin levels and the physical fitness of type 2 diabetic patients.Methods Sixty persons with type 2 diabetes were randomized into an electroacupuncture group (30 cases) and a transcutaneous stimulation group (30 cases).The patients were treated with either electroacupuncture or transcutaneous electrical stimulation of certain acupoints for a period of 2 months,30 minutes a day,5 times per week.At pre-treatment,post-treatment and follow-up,the indexes of 2 hour postprandial blood glucose (2hPG) and glycosylated hemoglobin (HbA1c),body mass index (BMI),waist-to-hip ratio (WHR) and percentage of body fat (PBF) were measured.Results After treatment,all of the above indexes had improved in both groups.At follow-up there was no longer any statistically significant difference compared with before treatment except for HbA1c levels in the electroacupuncture group.There was no statistically significant difference between the groups in terms of any index at any time point.Conclusion Transcutaneous electrical stimulation of the acupoints demonstrated effects similar to those of electroacupuncture in improving the blood glucose and glycosylated hemoglobin levels and the physical fitness of patients with type 2 diabetes.
