摘  要:目的:探讨重症监护病房(NICU)新生儿听力筛查模式及影响听力筛查的因素。方法:采用瞬态诱发耳声发射(TEOAE)和自动听性脑干反应(AABR)仪对13 315例新生儿,包括在出生后3~7d初筛、出生后42d复筛、未通过者,在出生后满3个月时实施全面听力学诊断评估的三步法筛查方案,进行常规听力初筛和复筛。结果:初筛未通过者5 151例,42d随访未通过者1 910例,出生后3个月随诊复查仍未通过的患儿1 167例于出生后6个月内经ABR和声导抗检查,最后确诊为先天性听力损失者642例,占总筛查例数的4.82%。结论:TEOAE联合AABR+全面听力学诊断评估的三步法筛查方案可能是NICU新生儿听力筛查的较佳模式。Objective:Explore the model of universal NICU newborns′hearing screening in high-risk neonates,preliminary understanding factor of hearing damage.Method:Transient evoked otoacoustic emissions(TEOAE)and automatic auditory brainstem response(AABR)were used to detect newborns′hearing in 13 315objects,that is newborns′hearing screening in NICU with TEOAE test who not pass,42days after will use AABR rescreening.Children′s Hearing Center of Guangxi Child Health Hospital will diagnose the newborns that did not pass in 3months.Result:In these 13 315newborns,5 151subjects who did not pass the initial screening,1910subjects who also did not pass after 42days,1 167subjects cannot pass the rescreening after 3months,642subjects were diagnosed congenital hearing impairment by Brainstem Auditory Evoked Potential Test,the rate is 4.82%.Conclusion:TEOAE and AABR are the suitable model of universal newborns′hearing screening in high-risk neonates.
