摘  要:探讨新生儿缺氧缺血性脑病脑组织低密度灶CT值在临床分级中的价值及其影响因素.随机抽取经CT检查并临床确诊的新生儿缺氧缺血性脑病79例,分析脑白质区低密度病灶CT值与临床分度、分娩后检查时间、分娩方式、胎龄的相关性. 临床分度轻度41例,中度29例,重度9例;CT值分别是(12.88±1.55) HU,(9.83±0.85) HU,(8.56±1.24) HU,差异具有统计学意义(P<0.01).分娩后1~3 d行CT检查65例,4~7d检查14例;CT值分别是(11.02±2.14) HU,(12.43±1.87) HU,差异具有统计学意义(P<0.05).顺产30例,剖宫产49例;CT值分别是(10.77 ±2.47) HU,(11.57±1.89) HU,差异无统计学意义(P>0.05).足月儿59例,早产儿20例;CT值分别是(11.51 ±2.20) HU,(10.55±1.88)HU,差异无统计学意义(P>0.05). 在适当时间测定的新生儿缺氧缺血性脑病脑组织低密度病灶CT值,可以用来评估脑组织损伤程度,作为临床诊断和病情分级的客观指标,具有重要的参考价值.Objective : To investigate the role of the low density lesion CT value of the neonatal hy- poxic ischemic encephalopathy in the clinical classification and its influencing factors. Methods: 79 infants with HIE confirmed by CT and clinical examination were randomly selected. The correlation between the CT value of low density lesions of cerebral white matter and clinical classification, inspection time after delivery, delivery mode, gestational age was analyzed. Results: The clinical classification showed that 41 cases were mild, 29 cases were moderate, 9 cases were severe. The CT values were (12.88 ±1.55) HU, (9.83 ±0.85) HU, (8.56 ±1.24) HUrespectively, the difference was statistically significant (P 〈 0.01 ). 65 cases were examined by CT in 3 days after birth, 14 cases in 4 -7 days; The CT values were (11.02 ±2.14) HU, (11.02 ± 1.87 ) HU respectively, the difference was statistically significant (P 〈 0.05). The CT value of 30 cases of natural labor was ( 10.77 ± 2.47) HU, the CT value of 49 cases of cesarean section was ( 10.77 ± 1.89) HU, the difference was not statistically significant (P 〉 0.05). The CT value of 59 full-term infants was ( 11.51 ± 2.20) HU, the CT value of 20 prematures was ( 10.55 ± 1.88 ) HU, the difference was not statistically significant ( P 〉 0.05 ). Conclusion : The CT value of HIE low-density lesions at the appropriate time was useful to assess the degree of brain tissue damage, as an objective indicator of clinical diagnosis and clinical classification.
