甘精胰岛素联合二甲双胍对老年2型糖尿病患者血糖、hs-CRP及MDA水平的影响

摘　　要：目的观察甘精胰岛素联合二甲双胍治疗老年2型糖尿病(T2DM)的临床疗效,探讨其对血糖、高敏C反应蛋白(hs-CRP)及丙二醛(MDA)水平的影响。方法将72例老年T2DM患者随机分为治疗组和对照组,每组36例,对照组皮下注射甘精胰岛素,治疗组在此基础上给予二甲双胍,疗程均为6周。比较2组治疗后的临床疗效,治疗前后血糖指标、hs-CRP及MDA水平的变化,并观察不良反应发生情况。结果治疗组总有效率显著高于对照组(P<0.05)。治疗后,2组空腹血糖(FBG)、餐后2 h血糖(2 hPG)及糖化血红蛋白(HbA1c)水平均较治疗前显著下降(P<0.01),其中治疗组降低幅度更为显著(P<0.05或P<0.01)。治疗后,2组hs-CRP及MDA水平均较治疗前显著降低(P<0.05或P<0.01),且治疗组治疗后hsCRP水平显著低于对照组(P<0.05)。结论甘精胰岛素联合二甲双胍能有效降低老年T2DM患者的血糖水平,缓解氧化应激程度,疗效显著。Objective To explore the clinical efficacy of glargine combined with metformin for old patients with type 2 diabetes mellitus（ T2DM） and its effect on the glucose,high-sensitivity C-reactive protein（ hs-CRP） and malondialdehyde（ MDA） levels. Methods Seventy-two old patients with T2 DM were randomly divided into treatment group and control group,36 cases in each group.The control group was subcutaneously injected glargine,while the treatment group was added metformin based on this. Both courses of treatment were 6 weeks. The clinical efficacy after treatment,changes of glucose-related indexes,hs-CRP and MDA levels before and after treatment were compared,and the incidence of adverse reactions was observed. Results The overall response rate of treatment group was obviously higher than in control group,with statistical significance（ P〈 0. 05）. After treatment,the levels of fasting blood glucose（ FBG）,2 h postprandial glucose（ 2hPG） and glycosylated hemoglobin（ HbA1c） in both groups went down significantly by comparison to treatment before（ P〈 0. 01）,in which the decreased range in treatment group was more significant（ P〈 0. 05 or P〈 0. 01）. Besides,hs-CRP and MDA levels in two groups after treatment were notably decreased（ P〈 0. 05 or P〈 0. 01）,and hs-CRP level in treatment group was markedly lower than in control group（ P〈 0. 05）.Conclusion Glargine combined with metformin can effectively decrease the glucose level in old patients with T2 DM,and alleviate the degree of oxidative stress,with significant efficacy.
