缬沙坦联合氨氯地平对老年高血压合并糖尿病患者血糖、血脂和血尿酸水平的影响

摘　　要：目的探讨缬沙坦联合氨氯地平治疗老年高血压合并糖尿病的临床疗效及其对血糖、血脂和血尿酸水平的影响。方法选取120例老年高血压合并2型糖尿病患者为研究对象,随机分为2组,每组60例,联合组给予缬沙坦联合氨氯地平口服,单用组氨氯地平口服。观察2组患者的临床疗效、治疗前后血脂、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素敏感指数(ISI)及血尿酸(UA)等指标的变化。结果联合组总有效率93.33%,单用组总有效率80.00%,组间比较具有显著差异(P<0.05);治疗前2组血脂、UA水平、FBG、FINS及ISI无显著差异(P>0.05),治疗后联合组上述指标较治疗前均有显著改善(P<0.01),单用组与治疗前无显著差异(P>0.05)。结论缬沙坦联合氨氯地平治疗老年高血压合并糖尿病的临床疗效优于单用氨氯地平,且可显著改善脂肪代谢及胰岛素敏感性。Objective To explore clinical efficacy of valsartan combined with amlodipine on the level of blood sugar,serum lipid and blood uric acid of elderly patients with hypertension complicated with diabetics. Methods A total of 120 cases with hypertension complicated with diabetics were selected and divided into 2 groups. Combined-used group was given oral valsartan combined with amlodipine and single-amlodipine group was given amlodipine. Clinical efficacy and indexes of serum lipid,fasting blood-glucose（ FBG）,fasting insulin（ FINS）,insulin sensitivity index（ ISI） and blood uric acid（ UA） were observed. Results Total effective rate of combined-used group was 93. 33% and single-amlodipine group was 80. 00%,there was significant difference between 2 groups（ P〈 0. 05）.there were no differences in the levels of serum lipid,FBG,FINS and ISI before treatment（ P〉 0.05）. And the above mentioned indexes were significantly changed after treatment（ P〈 0. 0 1）. there were no differences in single- amlodipine group before treatment（ P〉 0.05）. Conclusion The clinical effect of valsartan combined with amlodipine for treating elderly patients with hypertension complicated with diabetics be superior to single-amlodipine group and can obviously improve fat metabolism and insulin sensitivity.
