关节镜清理术治疗膝骨性关节炎的疗效观察

摘　　要：目的探讨关节镜下关节清理术治疗膝关节骨性关节炎疗效。方法将本院于2012年7月至2013年7月收治的39例膝关节骨性关节炎患者,进行关节镜下关节清理术。应用KSS评分对手术前后的膝关节进行评估,并进行统计学比较。结果术后随访8~12个月,平均9.3个月。39例患者末次随访时膝关节评分为（40.74±2.26）、功能评分为（67.37±6.74）、总分为（108.11±8.08）,与术前比较差异均有统计学意义（P〈0.05）。根据KSS评分标准评价患者疗效：优19例,良13例,可5例,差2例,优良率为82.10%。影像学表现为晚期的5例患者,术后KSS评分3例为可、2例为差。结论应用关节镜进行膝关节清理术对治疗膝关节骨性关节炎具有满意的疗效,影像学表现为晚期的患者不建议进行关节镜下关节清理术。Objective To investigate the curative effect of arthroscopic debridement for the knee osteoarthritis. Methods From July 2012 to July 2013,39 patients with osteoarthritis were treated by arthroscopic debridement in our hospital. KSS score were used to evaluate knee joints before and after operations and the results were statistically analyzed. Results In postoperative follow-up 8 ~ 12 months,9. 3 months on average,the knee scores of the 39 patients at last follow-up were（40. 74 ± 2. 26）,（67. 37 ± 6. 74）,and the total score is（108. 11 ± 8. 08）,and the data was statistically significant compared with that before operation（P〈 0. 05）. According to the KSS evaluation criteria,19 patients reached excellent,13 good,5 fair and 2 bad. The excellent and good rate was82. 10%. Of 5 cases of advanced patients of imaging findings,according to postoperative KSS score,3 cases are fair and 2 cases were poor. Conclusions Arthroscopic debridement on knee osteoarthritis proves to satisfactorily effective to the treatment of knee osteoarthritis. But arthroscopic debridement was not recommended for patients diagnosed as at the late stage through imaging findings.
