依达拉奉对中、重型颅脑损伤患者脑水肿及预后的影响

摘　　要：目的观察依达拉奉对中、重型颅脑损伤(TBI)患者脑水肿及预后的影响。方法选取2011年2月—2014年2月住院治疗的124例中、重型TBI脑水肿患者,随机分成治疗组63例、对照组61例。对照组给予常规治疗,治疗组在此基础上予依达拉奉静脉滴注。观察治疗前及治疗后第3、7、14、28天的脑水肿面积、治疗第28天的格拉斯哥昏迷量表(GCS)评分及随访6个月后的Glasgow预后评分(GOS)。结果 2组治疗后第3、7天脑水肿面积均较治疗前显著扩大(P<0.01);第14、28天脑水肿面积显著减小(P<0.01);治疗组显著小于对照组(P<0.01)。治疗第28天,2组GCS评分均较治疗前显著升高(P<0.01);治疗组显著高于对照组(P<0.01)。随访6个月后,治疗组GOS评分良好率显著高于对照组(P<0.05)。结论依达拉奉可显著减轻中、重型TBI患者的脑水肿,改善预后。Objective To observe the influence of edaravone on hydrocephalus and prognosis of patients with moderate and severe traumatic brain injury（TBI）. Methods 124 patients with hydrocephalus due to moderate and severe TBI admitted were randomly divided into treatment group（n =63） and control group（n = 61）. Routine treatment was given to control group,on which basis edaravone was intravenously injected to treatment group. Hydrocephalus volumes before treatment and 3,7,14 and 28 days after treatment,Glasgow Coma Scale（GCS） scores 28 days after treatment as well as Glasgow Outcome Scale（GOS） scores 6 months after follow up were observed. Results Hydrocephalus volumes 3 and 7 days after treatment extended significantly than those before treatment in both groups（P〈0. 01）,decreased apparently 14 and 28 days after treatment（P〈0. 01）,and were smaller in treatment group than in control group（P〈0. 01）. 28 days after treatment,GCS scores increased significantly in both groups than those before treatment（P〈0. 01） and were higher in treatment group than in control group（P〈0. 01）. 6 months after follow up,GOS score was better in treatment group than in control group（P〈0. 05）. Conclusion Edaravone can significantly reduce hydrocephalus in patients with moderate and severe TBI and improve the prognosis.
