基于递归系统模型的农村老年人直接住院费用影响因素分析

摘　　要：目的旨在了解各因素对农村老年人直接住院费用影响。方法对山东省某县级市参合患者住院资料库中60岁及以上农村老年人医疗费用进行调查,采用递归系统模型分析各相关因素对直接住院费用的影响。结果研究结果发现医院级别、是否手术等指标对住院费用的影响不仅存在直接效应而且存在间接效应,且与住院费用呈正相关,说明以上指标既直接引起住院费用的增加,也可以通过影响住院天数而影响住院费用;而年龄、是否高血压等指标对住院费用仅有间接效应,说明以上指标仅间接影响住院费用。结论该研究通过分解影响因素的不同作用层次,寻找其内在联系,为控制费用不合理增长提供有意义的循证依据和量化参考。Objective To understand the affect of various factors on direct hospital costs of the elderly in rural areas. Methods To investigate the hospital costs of the participation of rural elderly patients who aged 60 and above in hospital databases on a county-level city in Sbandong Province, and to use the recursive system model to analyze the impact of relevant fac- tors on the direct hospital costs. Results The results shows the indexes such as the hospital level, whether the operation, has not only effect and indirect effect to influence hospitalization cost, it is also positively related associated with hospitalization cost. The indexes above is directly caused by the increased cost of hospitalization, but also can be affected by the length of hospital stay to hospital cost; the indexes such as age, whether hypertension, has only indirect effect to influence hospitalization cost. Conclusion Above all decomposing the different role factors and looking for its internal relations, this study propose the evidence-based and quantitative reference for control unreasonablecost growth.
