住院2型糖尿病患者糖尿病周围神经病变相关因素分析

摘　　要：目的探讨住院2型糖尿病患者糖尿病周围神经病变(DPN)的相关因素。方法收集219例2型糖尿病住院患者的临床资料,根据是否合并DPN,分为DPN组(153例)及非DPN组(66例),比较两组患者的临床资料,采用Logistic回归分析探讨住院2型糖尿病患者DPN的相关因素。结果与非DPN组相比,DPN组年龄高、糖尿病病程长(P<0.01),糖化血红蛋白(HbA1c)、空腹血糖、餐后2 h血糖、尿微量白蛋白排泄率(UAER)水平及合并糖尿病视网膜病变(DR)比例高(P<0.05或0.01)。Logistic回归分析显示DPN与年龄、HbA1c、UAER及合并DR有关(OR值分别为1.071、1.315、1.039、2.255)。结论年龄、HbA1c、UAER及合并DR是住院2型糖尿病患者DPN的重要相关因素,高龄、HbA1c高、UAER高及合并DR的住院2型糖尿病患者可能更易发生DPN。Objective To explore relevant factors of diabetic peripheral neuropathy （DPN） in hospi- talized patients with type 2 diabetes mellitus. Methods A total of 219 patients with type 2 diabetes were recruited into this study and divided into the DPN （n = 153 ） and non-DPN groups （n = 66） according to peripheral neuropathy. Basic clinical data were statistically compared between two groups. Logistic regression analysis was adopted to identify relevant factors of DPN. Results Compared with the non-DPN group, patients in the DPN group were older and had a longer duration of diabetes （ both P 〈 0. 01 ）. The levels of glycosylated hemo- globin （ HhAI,： ） , fasting blood glucose （FPC） , 2 hour postprandial blood glucose （2hPG） , urinary albumin excretory rate （UAER） and the incidence of diabetic retinopathy （ DR ） in DPN patients were significantly higher than those in non-DNP counterparts （P 〈 0. 05 or P 〈 0. 01 ）. Logistic regression analysis revealed that age, HbAmc, UAER and complication with DR were significantly correlated with DPN （OR were 1. 071, 1. 315, 1. 039 and 2. 255, respectively）. Conclusions Age, HhAlc, UAER and complication with DR are pivotal relevant factors of DPN in hospitalized patients with type 2 diabetes mellitus. Diabetic patients who are older, have higher levels of HbAtc and UAER and are complicated with DR have a high incidence of DPN.
