血清β-HCG、孕酮联合阴道超声检测对早期异位妊娠的诊断价值

摘　　要：目的探讨血清绒毛膜促性腺激素(β-HCG)、孕酮(P)联合阴道超声检测对早期异位妊娠的临床诊断价值。方法对51例异位妊娠患者(异位妊娠组),48例正常早孕者(正常妊娠组),56例健康体检者(健康对照组)分别检测其血清β-HCG、P水平和子宫内膜厚度;比较血清β-HCG、P检测、阴道超声检测及三项联合检测的敏感性、特异性、准确性。结果血清β-HCG、P水平及子宫内膜厚度正常妊娠组均明显高于异位妊娠组(P<0.01),健康对照组又明显低于异位妊娠组(P<0.01)。而三项联合检测的敏感性、特异性和准确性分别提高至94.1%、95.7%和74.2%,高于各项的单独检测(P<0.05)。结论联合检测血清β-HCG、P和阴道超声有利于提高异位妊娠诊断的敏感性、特异性和准确性,对异位妊娠的早期诊断有重要意义。Objective To investigate the value of serum beta human chorionic gonadotrophin （β-HCG） , progesterone and trans- vaginal ultrasonography in the diagnosis of early ectopic pregnancy. Methods All the subjects were divided three groups, i. e. Group A （ early ectopic pregnancy group, n = 51 ） , Group B （ early normal pregnancy group, n = 48 ） and Group C （ healthy control group, n = 56）. The levels of serum β-HCG and progesterone were analyzed, and vaginal endometrial thickness was measured by ultrasonography. Comparison was done on the aspects of sensitivity, specificity and accuracy of β-HCG and progesterone, in combi- nation of transvaginal uhrasonography. Results Compared with that of Group A, the levels of β-HCG and progesterone and the va- ginal endometrial thickness of Group B were significantly increased （ P 〈 0.01 ） ; the parameters were lower in Group C than in Group A （P 〈 0.01 ）. The sensitivity, specificity and accuracy rate （94.1% , 95.7% and 74.2% ） of the combined application of β-HCG and progesterone detection and transvaginal uhrasonography were significantly higher than that of single application （P 〈 0.05 ）. Conclusion Combined detection of serum β-HCG and progesterone and transvaginal ultrasonography will improve the sen- sitivity, specificity and accuracy of early ectopic pregnancy and it is good for the early diagnosis of the normal condition.
