广痛消泡沫气雾剂对痔术后解痉止痛作用的临床研究

摘　　要：目的研究广痛消泡沫气雾剂对痔切除术后的止痛效果。方法将88例III,IV期混合痔患者随机分为治疗组28例、对照组30例和空白组30例,行痔外剥内扎术。术后换药分别予广痛消泡沫气雾剂、0.2%硝酸甘油软膏和凡士林软膏,评价术后静息及排便时肛门疼痛、肛管压力、止痛药物的应用及药物不良反应。结果术后第2-7天治疗组与对照组肛门静息疼痛评分显著小于空白组（P〈0.05）;术后第2-4天3组排便时疼痛评分有统计学差异（P〈0.05）;术后第5天治疗组与对照组肛管压力显著小于空白组（P〈0.05）;术后第2,3天空白组止痛药物的应用数量多于其他2组（P〈0.05）;术后对照组头痛、头晕发生率显著高于其他2组（P〈0.05）。结论广痛消泡沫气雾剂能够安全有效地降低术后肛管静息压,缓解肛门内括约肌痉挛,减轻术后肛门疼痛。Objective To study the analgesic effect of Guangtongxiao Foam Aerosol on pain after Milligan-Morgan operation.Methods The patients with stage Ⅲ or stage Ⅳ mixed hemorrhoid（n=88） were randomly divided into treatment group（n=28）,control group（n=30） and blank group（n=30） and given Milligan-Morgan operation.After the operation,all groups were respectively given Guangtongxiao Foam Aerosol,0.2%nitroglycerin ointment and Vaseline ointment.The anal pain and anorectal pressure during rest and defecation,administration of analgesic drugs and adverse reactions were reviewed after the operation.Results After the operation for 2 d to 7 d,the scores of anal rest pain were significantly lower in treatment group and control group than those in blank group（P〈0.05）.The pain scores during defecation had statistical difference among 3 groups after the operation for 2 d to 4 d（P〈0.05）.The anorectal pressure was significantly lower in treatment group and control group than that in blank group（P〈0.05） after the operation for 5 d.The dosage of analgesic drugs was higher in blank group than that in other 2group（P〈0.05） on the 2nd d and 3rd d after the operation.The incidence rates of headache and dizziness were higher in control group than those in other 2 groups（P〈0.05） after the operation.Conclusion Guangtongxiao Foam Aerosol can effectively and safely reduce rest anorectal pressure,relax the spasm of anal sphincter and relieve postoperative anal pain.
