摘 要:目的:探究磷脂酰肌醇-3激酶δ的表达与妊娠期糖尿病(GDM)的相关性,为临床治疗GDM提供理论依据。方法:选取2012年1月~2013年7月在河北医科大学第二医院行产前检查的孕妇166例,其中正常孕妇44例为NGT组、GDM孕妇122例为GDM组,测量并记录两组孕妇年龄、身高、体重、孕周,并计算BMI指数;采集孕妇的血液样品并收集血液中的白细胞,通过生化实验检测血糖、血脂、空腹胰岛素(FIN)、胆固醇、C反应蛋白(CRP)、高密度脂蛋白(HDL)等指标水平,并计算稳态模型β细胞功能指数(HOMA-B)以及胰岛素抵抗指数(HOMA-IR);采用RT-PCR方法检测PIK3CD mRNA在白细胞内的表达水平。结果:两组孕妇年龄、身高、体重、孕周、BMI值等指标比较,差异无统计学意义(P〉0.05),GDM组HDL-C含量明显低于NGT组,空腹1 h和2 h后的葡萄糖浓度(FPG)及糖化血红蛋白(HbA1C)含量均明显高于NGT组,两组比较,差异有统计学意义(P〈0.05);两组血甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、血清总胆固醇(TC)、CRP、HOMA-IR以及HOMA-B等指标比较,差异无统计学意义(P〉0.05)。GDM组白细胞内的PIK3CD mRNA是NGT组的1.92倍。结论:GDM患者白细胞内的PIK3CD过量表达可引起HDL-C含量减少,并导致血糖浓度增加。Objective: To explore the correlation between phosphatidylinositol 3 - kinase δ expression and gestational diabetes mel- litus (GDM) , provide a theoretical basis for clinical treatment of GDM. Methods: A total of 166 pregnant women r/eceiving prenatal exami- nation in the Second Hospital of Hebei Medical University from January 2012 to July 2013 were selected and divided into normal pregnancy group (44 pregnant women) and GDM group ( 122 GDM pregnant women) ; maternal ages, body heights, body weights and gestational weeks in the two groups were measured and recorded, body mass index (BMI) values in the two groups were calculated; the blood samples of pregnant women were abstracted and white blood cells were collected; blood glucose levels, blood lipids levels, fasting insulin levels, cholesterol levels, C -reactive protein (CRP) levels and high -density lipoprotein cholesterin (HDL- C) levels in the two groups were detected by biochemical tests; HOMA - 13 and HOMA -IR were calculated; RT -PCR was used to detect the expression level of PIK3CD mRNA in white blood cells. Results: There was no statistically significant difference in maternal age, body height, body weight, gestational weeks and BMI between the two groups (P 〉 0. 05 ) , HDL - C level in GDM group was statistically significantly lower than that in normal pregnancy group ( P 〈 0.05 ) , one - hour and two - hour fasting blood glucose and glycosylated hemoglobin ( HbA1 C) levels in GDM group were statistically significantly higher than those in normal pregnancy group (P 〈 0. 05) ; there was no statistically significant difference in levels of tri- glyceride, low - density lipoprotein cholesterin ( LDL - C), total cholesterol, CRP, HOMA - IR and HOMA - 13 between the two groups (P 〉0. 05). PIK3CD mRNA in white blood cells of GDM group was 1.92 times more than that of normal pregnancy group. Conclusion: Over - expression of PIK3CD in white blood cells of GDM patients can induce decrease of HDL
