室性早搏危险分层预测阵发室性心动过速的分析

摘　　要：目的分析室性早搏(室早)危险分层各因素对预测阵发室性心动过速(阵发室速)的价值。方法回顾60例24 h动态心电图阵发室速组的检查结果,另选60例24 h动态心电图无阵发室速者为对照组,分析阵发室速患者的室早指数、室早易损指数、Myerburg室早分级和Schamaroth室早分级等室早危险分层因素,比较各因素发生阵发室速的概率,对比阵发室速组与无阵发室速组室早危险分层各因素的差异。结果室早指数、室早易损指数和Schamaroth病理室早中QRS振幅与QRS时限在阵发室速组与无阵发室速组间比较,差异无统计学意义(P>0.05);Schamaroth病理室早中QRS波切迹、不规则与ST-T改变和Myerburg室早分级2组比较,差异有统计学意义(P<0.05)。结论室早危险分层各因素中Myerburg室早(形态)可以预测阵发室性心动过速的发生。室早指数、室早易损指数对阵发室性心动过速的预测价值不明显。Objective Toanalyzethevalueoftheriskstratificationfactorsofprematureventricu-larcontraction（PVC）inpredictingparoxysmalventriculartachycardia（PVT）.Methods Accord-ing to the results of 24-hour ambulatory electrocardiography,60 cases with PVT were enrolled as PVT group and another 60 cases without PVT as the control group.For patients with PVT,their PVC index,PVC vulneralbe index,and the factors in Myerburg＇s risk stratification of PVC and Schamaroth＆#39;s risk stratification of PVC were analyzed.As far as the above factors were concerned, the incidence rates of PVT were compared.Between the two groups,the differences of factors in risk stratificationofPVCwerealsomadeacomparison.Results Therewasnostatisticalsignificancein the differences of PVC index,PVC vulneralbe index,and QRS amplitude and QRS duration in Schamaroth＇s pathological PVC between the two groups.However,the differences of QRS wave in-cisure,irregularity and ST-T changes in Schamaroth＇s pathological PVC,as well as Myerburg＇s risk stratificationofPVCwerestatisticallysignificant.Conclusion Amongvariousfactorsinriskstrati-fication of PVC,Myerburg＇s PVC（forms）is the most valuable in predicting the occurrence of PVT. The values of PVC index and PVC vulneralbe index in predicting PVT failed to show up.
