子宫内膜搔刮术对辅助生殖患者妊娠结局的影响

摘　　要：目的探讨子宫内膜搔刮术对接受辅助生殖助孕患者妊娠结局的影响。方法回顾性分析2012年1月至2013年8月于南京妇幼保健院生殖医学中心行鲜胚移植及冻融胚胎移植的患者,内膜形态异常分为搔刮组和未搔刮组,内膜形态正常为对照,比较一般情况及妊娠结局。结果 3组患者在年龄、不孕年限、移植胚胎数、异位妊娠率及流产率上比较差异均无统计学意义(均P>0.05);在鲜胚移植及冻胚移植的患者中,内膜未搔刮组、内膜搔刮组患者子宫内膜厚度显著高于对照组,t值分别为0.005/0.001(鲜胚);0.036/0.009(冻胚),均P<0.05;内膜搔刮组患者临床妊娠率显著高于未搔刮组(鲜胚x^2=0.861;冻胚x^2=4.837,均P<0.05),与内膜形态正常组妊娠率无明显差异(P>0.05)。结论经子宫内膜搔刮处理后,可改善患者子宫内膜厚度及容受性,提高不孕患者的临床妊娠率。Objective To explore the influence of endometrial curettage on the pregnant outcomes in patients with assisted reproductive technology. Methods Retrospective analysis was performed on fresh embryo transfer cycles and frozen-thawed embryo transfer cycles in department of reproduction of Nanjing Maternity and Child Health Hospital from January 2012 to August 2013. Patients with abnormal endometrial morphology were divided into curettage group and non-scraping group, and patients with normal endometrium were recruited in control group. The general information and pregnant outcomes were compared among three groups. Results There was no significant difference in age, year of infertility, number of embryo transfer, ectopic pregnancy rate and abortion rate among three groups （ all P 〉 0.05 ）. The endometrial thickness of two groups was significantly greater than the control group [ t = 0. 005/0. 001 （ fresh embryo transfer）, t = 0. 036/0. 009 （frozen-thawed embryo transfer）, both P 〈 0.05 ]. The clinical pregnancy rate in curettage group was higher than in non-scraping group [x2 = 0. 861 （fresh embryo transfer）, 2＂2 = 4. 837 （frozen-thawed embryo transfer）, both P 〈 0.05 ）, but it was not significantly different from that of the control group （P 〉 0.05 ）. Conclusion Endometrial curettage may improve the thickness and receptivity of endometrium and improve clinical pregnancy rate.
