摘 要:目的探讨血清β2微球蛋白(β2-MG)和胱抑素C(Cys C)检测在新生儿高胆红素血症肾损害诊断中的价值。方法检测72例高胆红素血症患儿(观察组)及30例非高胆红素血症新生儿(对照组)的血清总胆红素(TBiI)、β2-MG、Cys C、肌酐(Cr)和尿素氮(BUN)的含量,比较两组之间及观察组轻度、中重度患儿之间的检测结果,比较观察组治疗前后血清TBil、β2-MG、Cys C水平和治疗后观察组和对照组血清β2-MG、Cys C水平。结果观察组血清TBil、β2-MG、CysC水平均高于对照组,差异有统计学意义(t值分别为42.71、41.10和8.47,均P<0.05);观察组轻度患儿TBil、β2-MG、CysC水平均低于中重度患儿,差异有统计学意义(t值分别为9.96、14.16和8.08,均P<0.05);治疗后观察组血清TBil、β2-MG、CysC水平均较治疗前明显下降,差异有统计学意义(t值分别为65.46、54.21和14.18,均P<0.05);血清β2-MG、CysC水平与TBil水平成正相关(r值分别为0.81和0.53,均P<0.05),BUN、Cr水平与TBil水平无相关性(r值分别为0.21和0.16,均P>0.05)。结论血清β2-MG、CysC水平检测能够准确反应肾脏功能损伤情况,可作为高胆红素血症患儿肾损伤的敏感的准确指标,对病情的评估及临床疗效的判断也具有重要价值。Objective To investigate the value of serum β2-microglobulin (β2-MG) and cystatin C (CysC) in diagnosing renal impairment in newborns with hyperbilirubinemia. Methods Serum total bilirubin (TBil), β2-MG, CysC, creatinine (Cr) and blood urea nitrogen (BUN) contents of 72 newborns with hyperbilirubinemia (observation group) and 30 newborns without hyperbilirubinemia (control group) were detected, and the above results were compared between two groups and between the mild, moderately severe patient of the observation group. Serum TBil, β2-MG and CysC levels of the observation group before and after treatment were compared, and serum β2-MG and CysC levels of the two groups before and after treatment were compared. Results The serum TBil, β2-MG and CysC levels of the observation group were higher than the control group ( t value was 42.71,41.10 and 8.47, respectively, all P 〈 0.05 ). TBil, β2-MG and CysC levels of the mild patients in the observation group were lower than those of the moderately severe patients ( t value were 9.96, 14.16 and 8.08, respectively, all P 〈 0.05). The serum TBil, β2-MG and CysC levels were significantly decreased after treatment (t value was 65.46, 54. 21 and 14.18, respectively, all P 〈0.05). The serum β2-MG and CysC levels were positively correlated with TBil level(r value was 0.81 and 0.53, respectively, both P 〈 0.05 ) , but BUN and Cr levels had no correlation with TBil level( r value wad 0.21 and 0.16, respectively, both P 〉0.05 ). Conclusion The detection of serum β2-MG and CysC levels can accurately reflect the renal function damage, and they can be considered as the accurate and sensitive indexes for renal impairment in newborns with hyperbilirubinemia. They also have an important value for evaluation of the disease and judgment for clinical curative effect.
