摘  要:目的探讨血浆醛固酮与慢性肺源性心脏病肺动脉高压的关系。方法选择90例慢性阻塞性肺病(COPD)患者分为COPD合并肺动脉高压组(n=30)、慢性肺源性心脏病肺心功能代偿期组(n=30)、慢性肺源性心脏病肺心功能失代偿期组(n=30),采用放射免疫分析法测定3组患者的血醛固酮水平、多普勒超声检测肺动脉压力。结果慢性肺源性心脏病肺心功能失代偿组血浆醛固酮浓度为(124.78±27.64)pg/ml,显著高于COPD合并肺动脉高压组(79.91±20.76)pg/ml及慢性肺源性心脏病肺心功能代偿期组(83.56±28.45)pg/ml,P<0.05;血浆醛固酮浓度与肺动脉高压呈线性正相关(r=0.514,P<0.05),与血氧分压呈负相关(r=-0.248,P=0.019)。结论醛固酮参与慢性肺源性心脏病患者肺动脉高压的发展。Objective To investigate the relationship between the plasma aldosterone levels and pulmonary hypertension in patients with chronic cor pulmonale. Methods A total of 90 chronic obstructive pulmonary disease (COPD) patients were divided into three groups, including COPD with pulmonary hypertension (n = 30 ), compensated chronic cor pulmonale (n = 30 ) and decompensated chronic cor pulmonale groups ( n = 30). Plasma aldosteron was measured by radioimmunoassay and pulmonary pressure was detected by Doppler ultrasound in the subjects above. Results The result showed that the level of plasma aldosteron was significantly higher in decompensated chronic cor pulmonale groups ( 124.78±+ 27.64 ) pg/ml than that in COPD and pulmonary hypertension group ( 79.91 ± 20. 76 ) pg/ml, compensated chronic cor pulmonale group ( 83.56 ±28.45 ) pg/ml, P 〈 0.05. plasma aldosteron levels was positively correlated with pulmonary hypertension( r = 0. 514, P 〈 0. 05 ), negatively correlated with partial pressure of oxygen ( r = -0. 248, P = 0. 019 ). Conclusions Plasma aldosteron participates in the development of pulmonary hypertension in patients with chronic cor pulmonale.
