摘  要:目的探讨脉冲多普勒超声心动图对胎儿心律失常的诊断价值。方法产前发现心律失常胎儿136例,行胎儿超声心动图检查,并获取肺动、静脉多普勒血流频谱,观察频谱间节律变化,对心律失常进行诊断及分类。结果脉冲多普勒超声心动图检出136例心律失常胎儿,其中房性早搏79例(58.1%),室性早搏27例(19.8%),心动过速24例[其中窦性心动过速13例(9.6%),室上性心动过速11例(8.1%)],窦性心动过缓2例(1例为单心室和1例为完全性房室间隔缺损均引产),心房扑动4例,其余均未见先天性心血管畸形及其他畸形。产后超声心动图未见明显心脏畸形及心律时常。结论脉冲多普勒超声测量肺动、静脉血流频谱方法能快速、准确地分类诊断胎儿心律失常。Objectives To investigate the diagnosis value of pulse Doppler echocardiography in prenatal diagnosis for fetal arrhythmia. Methods One hundred and thirty-six fetuses with arrhythmia were examined by prenatal fetal echoeardiography. The different kinds of fetal arrhythmias were diagnosed and classified according to pulse Doppler flow inaging frequency spectrum in pulmonary arteries and pulmonary veins. Results One hundred and thirty-six fetus with arrhythmia were found by pulse Doppler echocardiography,including 79 cases of atrial premature beats,27 cases of ventrieular premature beats,24 cases of tachyeardia (13 cases of sinus tachycardia and 11 cases of supraventricular tachycardia) ,2 cases of sinus bradycardia ( 1 cases of single ventricle and 1 cases of complete atrioventricular septal defect, both were induced labor ) , and 4 cases of atrial flutter. The rest eases were not associated with congenital cardiovascular malformations or other deformities. Echocardiography showed there was no obvious cardiac abnormality or arrhythmia after birth. Conclusion The pulse Doppler echocardiography to detect flow inaging frequency spectrum in pulmonary arteries and pulmonary veins can diagnose and classify fetal arrhythmia quickly and accurately.
