电针治疗膝骨关节炎疗效及对股四头肌表面肌电信号的影响

摘　　要：目的观察电针治疗膝骨关节炎的临床疗效及对股四头肌表面肌电信号的影响。方法将72例膝骨关节炎患者随机分为两组,治疗组38例采用电针治疗,对照组34例采用口服盐酸氨基葡萄糖片,比较两组临床疗效;应用表面肌电仪检测治疗前后患者患侧股四头肌中的股直肌、股外侧肌和股内侧肌的积分肌电值(IEMG)和中位频率(MF)的变化;应用Lequesne评分指数评价治疗前后患者膝关节功能障碍情况。结果治疗组总有效率为86.8%,对照组为67.6%,治疗组优于对照组(P<0.05);治疗组股直肌和股外侧肌的积分肌电值和中位频率的改善优于对照组(P<0.05);治疗组股内侧肌的积分肌电值和中位频率较对照组无明显改变(P>0.05);治疗组Lequesne指数积分的改善优于对照组(P<0.05);两种治疗方法均能改善股直肌、股外侧肌和股内侧肌的积分肌电值,中位频率及Lequesne指数积分(P<0.05)。结论电针治疗膝骨关节炎疗效明确,能增强股直肌与股外侧肌的力量和改善股直肌与股外侧肌的耐疲劳性,能明显改善患者膝关节功能。Objective To investigate the clinical efficacy of electroacupuncture in treating knee osteoarthritis and its effect onfemoral quadriceps surface electromyogram signals. Method Seventy-two patients with knee osteoarthritis were randomlyallocated to two groups. The treatment group received electroacupuncture and the control group took glucosamine hydrochloridetablets. The clinical therapeutic effects were compared between the two groups. Integrated electromyogram （IEMG） valuesand median frequencies （MF） for the rectus femoris, vastus lateralis and vastus medialis of the quadriceps femoris on the affectedside were measured using a surface electromyograph in the patients before and after treatment. Knee dysfunction was assessed usingLequesne score index in the patients before and after treatment. Result The total efficacy rate was 86.8% in the treatment groupand 67.6% in the control group. The effect was better in the treatment group than in the control group （P〈0.05）. Integratedelectromyogram values and median frequencies for the rectus femoris and vastus lateralis improved more in the treatment groupthan in the control group （P〈0.05）. Integrated electromyogram value and median frequency for the vastus medialis did notsignificantly change in the treatment group compared with the control group （P〉0.05）. The Lequesne index score improved more inthe treatment group than in the control group （P〈0.05）. Both treatments improved integrated electromyogram values and medianfrequencies for the rectus femoris, vastus lateralis and vastus medialis, and the Lequesne index score. ConclusionElectroacupuncture has a definite therapeutic effect on knee osteoarthritis. It can increase rectus femoris and vastus lateralis strength,build up rectus femoris and vastus lateralis fatigue resistance and significantly improve knee joint function in the patients.
