医院-社区-家庭联合采用VSD治疗压疮的临床研究

摘　　要：目的探讨医院-社区-家庭联合采用负压封闭引流技术(VSD)治疗模式的优越性及可行性。方法采用前瞻、对照设计的实验方法,将符合压疮临床分级标准的Ⅲ期、Ⅳ期压疮患者分为实验组(n=108)及对照组(n=74),实验组采用医院-社区-家庭综合VSD治疗方法,对照组采用传统的全程医院住院行VSD治疗,评价两组的疗效、费用及患者满意度。结果治疗6周后,实验组伤口缩小率为(55.3±13.5)%、伤口周围皮肤评分为(4.75±0.07)、肉芽组织评分为(4.89±0.07)、伤口愈合率为21.3%(23/108),与对照组比较差异无统计学意义(P>0.05);实验组治疗总费用为(13 253.2±492.7)元,低于对照组的(24 104.3±1 721.3)元,实验组患者满意度为95.4%(103/108),高于对照组的42.3%(33/74),差异均有统计学意义(P<0.05)。结论与传统的全程医院住院行VSD治疗模式相比,新型的医院-社区-家庭综合VSD治疗模式具有费用低、患者满意度高的优势,可大大减轻压疮患者的经济负担,改善患者生活质量。Objectlve To explore the superiority and feasibility of comprehensive hospital-community-family use of vacuum sealing drainage （VSD）. Methods Designed by foresight, contrast experimental method, the cases of stage Ⅲ or Ⅳ pressure ulcers were divided into experimental group （108 cases） and control group （74 cases）. The experimental group with hospital-community-family comprehensive treatment of VSD and the control group with traditional whole process of hospital treatment of VSD.Curative effect,cost and patient satisfaction were compared between two groups. Results There was no statistically significant difference between the experimental group and control group of the reduced rate of wound after 6 weeks VSD therapy, the surrounding skin wound score, granulation tissue score, and wound healing rate. The total cost of treatment of experimental group[RMB（13 253.2±492.7）] was lower than that of the control group [RMB（24 104.3±1 721.3）], and the satisfaction of experimental group（103/108,95.4%） was higher than that of the control group（33/74,42.3%）;the differences were statistically significant（P〈0.05）. Conclusion Compared with the whole process of hospital treatment of VSD, new type of comprehensive hospital-community-family VSD treatment model has the advantages of low cost, high patient satisfaction, greatly reduce the economic burden of patients, and betterquality of life of patients.
