回阳生肌膏对慢性皮肤溃疡形态变化的初步研究

摘　　要：目的：探讨回阳生肌膏治疗慢性皮肤溃疡腐肉、肉芽、脓液及组织HE染色形态变化规律。方法收集首都医科大学附属北京中医医院2012年11月-2013年11月40例局部辨证为寒湿凝滞证慢性疮面患者，随机分为两组，治疗组应用回阳生肌膏纱条，对照组按照常规方法进行。观察两组治疗前后疮面形态评分和治疗前后皮下组织HE染色变化情况。结果对照组总的评分治疗前为（29.1±5.7）分，治疗组为（28.4±6.1）分，差异无统计学意义（P〉0.05）；对照组治疗后为（25.7±5.4）分，与治疗前比较，差异有统计学意义（P〈0.05），治疗组为（22.9±6.8）分，与治疗前比较，差异有统计学意义（P〈0.05）。对照组与治疗组治疗后组间比较，差异有统计学意义（P〈0.05）。皮下组织HE染色观察，治疗前炎症细胞较多，内皮细胞、成纤维细胞较少且形态瘦小，分布散乱，胶原纤维淡染，治疗后炎症细胞减少，内皮细胞、成纤维细胞数量多形态饱满，分布规律，胶原纤维深染，治疗组较对照组有更好的愈合趋势。结论回阳生肌膏主要功效“温阳益气”，使慢性皮肤溃疡疮面由“阴”转“阳”，由“静”转“动”，从而加速疮面愈合。Objective To investigate the treatment of Huiyang Shengji Ointment to chronic skin ulcer carrion, granula-tion, pus and the change regulations of tissue HE staining. Methods 40 patients in Beijing TCM Hospital Affiliated to Capital Medical University from November 2012 to November 2013 whose local syndrome differentiation were cold damp stagnation syndrome of chronic wound, the subjects were randomly divided into two groups. The treatment group was used Huiyang Shengji Ointment gauze, the control group was taken conventional method. The wound surface mor-phology scoring and the two groups＇ changes of subcutaneous tissue HE staining before and after treatment were ob-served. Results The total score of the control group was （29.1±5.7） scores, and the treatment group was （28.4±6.1） scores before treatment, the difference between the two groups showed no significant difference （P 〉 0.05）; the total score of the control group was （25.7±5.4） scores after the treatment, compared with the control group, the difference was statistically significant （P〈0.05）. The total score of the treatment group was （22.9±6.8） scores, compared with the treat-ment group, the difference was statistically significant （P 〈 0.05）. The comparison between the control group and the treatment group showed that, the difference was statistically significant （P〈0.05）. The subcutaneous tissue HE staining showed that there were more inflammatory cells before treatment, endothelial cells and fibroblasts were less and thin, and they sprawled out in some areas, collagen fibers were dyed pale; there were less inflammatory cells after treatment, endothelial cells and fibroblasts were more and well-stacked, they distribute regularly, ollagen fibers were dyed dark. Healing trend was better in the treatment group than in the control group. Conclusion Warming Yang and Invigorating Qi, the main effect of Huiyang Shengji Ointment, can make chronic skin ulcer w ound turn ＂Yin＂ into ＂Yang＂, and turn ＂Static�
