成都郊县医院临床重点专科建设现状调查

摘　　要：目的调查分析成都市郊县医院临床重点专科建设的现状,为探索县级医院临床重点专科建设思路、评估标准及管理办法等提供参考。方法采用自制问卷调查了解成都市郊县医院临床重点专科建设情况,随机访谈成都市郊县医院领导、职能科长、临床科室主任、学科带头人对临床重点专科建设的建议,利用焦点小组讨论收集课题组成员意见,并使用SPSS 17.0统计学软件进行描述性分析。结果成都市郊县22家县级公立综合医院中有14家医院(占医院总数的63.64%)共建设了69个临床重点专科(省级5个、市级4个、县级29个、院级31个),分别分布在骨科、普外科、心血管内科、呼吸内科、消化内科等19个专业;只有郫县卫生局制定了县级临床重点专科建设管理办法和评估指标体系,22家县级公立综合医院均未制定院级临床重点专科评估标准。结论成都市郊县医院要提高对临床重点专科建设的认识,加大对临床重点专科建设的投入,改变临床科室的专业设置模式,制定适合县级医院的临床重点专科评估标准,规范临床重点专科的管理,增加省市级临床重点专科数量,充分发挥临床重点专科的能力辐射和技术带动作用。Objective To investigate the current situation of key clinical specialty construction in Chengdu rural hospi-tals, in order to provide a reference for the exploration of that in aspects of ideas, assessment standards and manage-ment methods. Methods Self-made questionnaires about the situation of clinical specialty construction were used, sug-gestions from chief leadership functions, the directors of clinical department and academic leaders were get by random interviews, members views were collected by using focus group discussion, and descriptive analysis were made by using SPSS 17.0. Results There were 14 in 22 （63.64%） county general hospitals set up a total of 69 key clinical specialties （5 provincial key clinical specialties, 4 municipal key clinical specialties, 29 county level key clinical specialties, 31 hospital level key clinical specialties）, distributed in 19 professionals, including the Department of Orthopedics, Gener-al Surgery, Cardiovascular Medicine, Respiration Medicine, Gastroenterology. Only the Pixian Health Bureau formulated the county-level key clinical specialty construction management methods and evaluation system, 22 county general hospitals did not draw up the academy-level key clinical specialty evaluation standards. Conclusion Chengdu City ru-ral hospitals should improve the understanding of key clinical specialty construction, increase the investment, change the major mode of clinical departments, make suitable key clinical specialty evaluation standards, standard the key clinical specialty management, increase the number of provincial and municipal key departments and give full play to the ability of the radiation and leading role.
