摘  要:目的观察白藜芦醇(Resveratrol)对心肌缺血再灌注损伤的保护作用及其具体机制。方法将36只SD大鼠随机分为3组:对照组、缺血再灌注组(I/R)、白藜芦醇组。制备离体心脏心肌缺血再灌注模型,分别测量:(1)复灌20和40 min时心功能指标:心率(HR)、左室舒张压(LVDP)、左室舒张末压(LVEDP)、心室内压最大变化速率(±dp/dtmax);(2)复灌40 min时心肌梗死面积;(3)复灌20和40 min时肌酸激酶(CK)和乳酸脱氢酶(LDH)活性;(4)复灌40 min时心肌组织中氨基末端激酶(Jun N-terminal kinase,JNK)的磷酸化水平。结果 (1)白藜芦醇明显提高心室内压最大变化速率(±dp/dtmax)(P<0.05),同时也明显改善LVEDP、LVDP等指标(P<0.05);(2)白藜芦醇减小心肌梗死面积(P<0.05);(3)再灌注40min时,与缺血再灌注组比,白藜芦醇明显降低乳酸脱氢酶(LDH)和肌酸激酶(CK);(4)白藜芦醇降低缺血心肌组织中磷酸化JNK的水平(P<0.05)。结论白藜芦醇可以改善缺血心肌的血流动力学,增加心肌收缩力,减小心肌梗死面积;其对离体心肌缺血再灌注的保护作用可能与JNK途径密切相关。Objective To investigate the protective effect of resveratrol on myocardial ischemia- reperfusion injury. Methods Thirty six rats were randomly divided into three groups:control group,ischemia- reperfusion injury(I /R) group and resveratrol group. These rat hearts subjected to 30 min ischemia and 40 min reperfusion. Heart rate( HR),left ventricular diastolic pressure(LVDP),left ventricular end- diastolic pressure(LVEDP) and the maximum change rate of intraventricular pressure( ± dp /dtmax) were recorded before ischemia and at 20 and 40 min of reperfusion. The coronary effulent were collected before ischemia and at 20 and 40 min of reperfusion to measure creatine kinase(CK) and lactate dehydrogenase(LDH) activity. The infarct size and Jun Nterminal kinase(JNK) activity were calculated at 40 min of reperfusion. Results Resveratrol enhanced LVEDP,LVDP and the maximum change rate of intraventricular pressure( ± dp /dtmax). The infarct size,CK and LDH activity were lower in Resveratrol group than that in ischemia and reperfusion group. And JNK activity was also lower in Resveratrol group. Conclusion Resveratrol has protective effect on isolated myocardial ischemia and reperfusion injury,and JNK signaling pathway may play an important role in the mechanism.
