布地奈德悬液雾化吸入对新生儿呼吸机相关性肺炎的影响

摘　　要：目的:探讨布地奈德悬液雾化吸入对新生儿呼吸机相关性肺炎(VAP)的影响。方法:选择新生儿科住院并需要机械通气的符合VAP诊断标准的78例新生儿,随机分为对照组和观察组,每组39例。对照组予氨溴索雾化吸入合用广谱抗生素,观察组在常规治疗基础上加用布地奈德悬液雾化吸入。比较2组肺部罗音消失时间、胸部X线片好转时间、血白细胞(WBC)及C反应蛋白(CRP)水平、有创通气时间及住院时间。结果:与对照组比较,新生儿VAP使用布地奈德悬液雾化吸入,可加快肺部罗音的消失及胸部X线片的好转(P<0.01和P<0.05),缩短有创通气时间及住院时间(P<0.01),2组患儿上机后第6天血WBC和CRP均较上机后第3天明显下降(P<0.01),但2组血WBC及CRP水平均无统计学意义(P>0.05)。结论:布地奈德悬液雾化吸入可缩短新生儿VAP的疗程,提高常规治疗的疗效。Objective：To explore the effects of budesonide nebulization inhalation on neonatal ventilator-associated pneumonia （VAP）.Methods：Seventy-eight neonates with VAP were randomly divided into the observation group and control group（39 cases each group）.The control group were treated with the nebulization inhalation of ambroxol and broad spectrum antibiotics,the observation group were treated with the nebulization inhalation of budesonide based on the routine treatment.The time of moist rales disappearance and chest improvement,the levels of leucocyte and C-reactine protein （CRP）,invasive ventilation time and length of hospital stay between two groups were compared.Results：Compared with the control group,the nebulization inhalation of budesonide in treating the neonatal VAP could accelerate the moist rales disappearance,improve the chest,shorten the invasive ventilation time and length of hospital stay（P ＜0.01 and P ＜0.05）.The levels of leucocyte and CRP at 6 days after treatment were significantly lower than those at 3 days after treatment in two groups（P ＜ 0.01）,the differences of leucocyte and CRP levels between two groups were not statistically significant （P ＞ 0.05）.Conclusions：The nebulization inhalation of budesonide can shorten the course of neonatal VAP and improve the curative effects of the conventional therapy.
