摘 要:目的了解川北医学院附属医院重症监护病房(ICU)革兰阴性菌的分离率及耐药情况。方法采用Bio-Merieux VITEK-32全自动细菌鉴定系统对细菌进行鉴定,琼脂稀释法进行药敏试验,双纸片表型确证试验检测产ESBLs菌株,结果用美国临床实验室标准化协会(CSLI)2006年公布的标准进行判定。结果 ICU分离454株革兰阴性杆菌,大肠埃希菌180株(占39.6%),鲍曼不动杆菌78株(占17.2%),阴沟肠杆菌59株(13.0%),铜绿假单胞菌51株(占11.2%),洋葱伯克霍尔德菌46株(占10.1%)为主。其中,超广谱β-内酰胺酶(ESBLs)大肠埃希菌和阴沟肠杆菌的检出率分别达54.5%和39.0%。且大多数革兰阴性杆菌对临床常用抗菌药高度耐药或多重耐药。结论该院ICU革兰阴性杆菌分离率高,临床耐药严重。Objective To investigate the isolating rate and drug resistance of Gram-negative bacterium isolated from ICU in Affiliated Hospital of North Sichuan Medical College. Methods The bacillus were evaluated by the system of Bio-Merieux VITEK-32. Agar dilution medthod was used for drug sensitivity test. ESBLs producing strains were detected by double paper phenotype corroborate test. The results was determined with the standard published by American Clinical Laboratory Standards Institute(CSLI)in 2006. Results 454 gram negative bacteria isolated from ICU,escherichia coli 180(39. 6% ), acinetobacter baumannii 78(17. 2% ),enterobacter cloacae 59(13. 0% ),pseudomonas aeruginosa 51(11. 2% ),burkholderia cepacia 46(10. 1% ). The extended-spectrum β-lactamases(ESBLs)escherichia coli and enterobacter cloacae detection rate was 54. 5% and 39. 0% respectively. The great height resistant rate and multiple drug resistance were showed in the most Gram-negative bacteria. Conclusion The high isolating rate of Gram-negative bacterium showed in ICU and the drug resist-ance was very serious.
