摘  要:目的分析总结应用镍钛记忆合金环抱器治疗肋骨骨折的手术技巧,探讨固定失败的原因及对策。方法回顾性分析137例使用镍钛记忆合金环抱器治疗肋骨骨折患者的术后临床资料。结果随访至术后第6个月,137例患者中成功随访112例,手术成功106例。另外6例固定失败,其中断端出现明显错位3例,切口愈合不良2例,发生重度排斥反应1例。结论镍钛记忆合金环抱器行内固定治疗为目前肋骨骨折的一种较为理想的手术方式。虽然近年来广泛应用后出现少量失败的情况,但较传统的手术固定方式有明显的进步。术中选择合适的环抱器型号,注意保护伤区软组织及骨膜,术后有效引流,可进一步提高成功率。Objective To analyze and summarize the surgical technique of Ni-Ti shape memory alloy embracing fixator for rib fracture fixation, and investigate the failure reasons and countermeasures of fixation. Methods The postoperative clinical data of 137 patients with rib fracture treated by fixation using Ni-Ti shape memory alloy were analyzed retrospectively. Results The patients were tollowed-up for 6 months after operation, and 112 patients had successful follow-up, 106 cases had successful operation. Six cases had failed fixation, including 3 cases of dislocated obviously after operation, 2 cases of poor wounds healing, and 1 cases of severe foreign body rejection. Conclusion Ni-Ti shape memory alloy embracing fixator is an ideal method for the internal fixation of fractured ribs. Although there were a few failure cases in recent years after widely application, it has significant improvement compared with traditional fixation method. The success rate of this operation carl be further improved by selecting appropriate type of embracing fixator, protecting soft tissue and periosteum of the operation area and giving effective drainage after the surgery.
