老年高血压患者不同中医证型夜间血压水平分析

摘　　要：目的研究老年高血压患者不同中医证型夜间动态血压水平差异,及其与各证型之间的关系。方法选择2013年7月—2014年1月中国中医科学院广安门医院心内科住院的老年高血压患者166例,其中阴虚阳亢组32例,脾肾阳虚组20例,痰瘀互阻组38例,气阴两虚组54例,气虚血瘀组22例。比较每两组间生化指标及动态血压参数情况。结果与其他各组患者比较,脾肾阳虚组患者尿酸水平最高,胆固醇、低密度脂蛋白胆固醇水平最低,夜间收缩压、夜间舒张压水平最低,其他参数各组间比较差异无统计学意义(P>0.05)。结论脾肾阳虚证与夜间血压水平关系密切,夜间血压水平较低或许可以成为诊断老年高血压患者脾肾阳虚证的客观参考依据之一。Objective To study the level differences of nighttime ambulatory blood pressure among different TCM syndromes in aging hypertension patients, and their relationship with different syndromes. Methods From July of 2013 to January of 2014, 166 aging hypertension patients were recruited in Department of Cardiology of Guang＇ anmen Hospital of China Academy of Chinese Medical Sciences, and divided into a group of yin-deficiency and yang-excess （32 cases）, a group of spleen-kidney yang-deficiency （20 cases）, a group of phlegm and blood stasis obstruction （38 cases）, a group of qi-yin deficiency （54 cases） and a group of qi- deficiency and blood-stasis （22 cases）. The biochemical indexes and ambulatory blood pressure were compared among different groups. Results Compared with other groups, the group of spleen-kidney yang-deficiency had highest uric acid, lowest cholesterol, low density lipoprotein, nighttime systolic and diastolic pressure; the rest parameters were not statistically significant between different groups （P 〉 0. 05 ）. Conclusion The syndrome of spleen-kidney yang-deficiency has a close relationship with nighttime blood pressure; lower level of nighttime blood pressure may be considered as one of the objective referencesfor aging hypertension withspleen-kidney yang-deficiency syndrome.
