摘  要:目的研究中药清毒栓及干扰素α-2b对宫颈高危型HPV感染的疗效,及对阴道局部免疫环境的调节作用,探讨其可能的免疫调节机制。方法对符合宫颈高危型HPV感染诊断标准患者按照其治疗意愿,分为清毒栓治疗组(清毒栓组)、干扰素α-2b治疗组(干扰素组)、随访组,治疗3个疗程,在完成治疗后的第1个月随访,于治疗前及随访时收集阴道灌洗液。ELISA法检测阴道灌洗液中干扰素-γ(IFN-γ)、细胞因子白介素-10(IL-10)含量。分析药物治疗HR-HPV感染的临床疗效,探讨药物治疗、疗效及阴道免疫微环境改变三者的相关性。结果清毒栓组、干扰素组及随访组,HPV转阴率分别为38.46%、30.00%、20.00%,疗效差异无统计学意义(P>0.05)。清毒栓及干扰素α-2b治疗后,阴道灌洗液中IFN-γ水平升高(P<0.05),治疗前后IL-10比较差异无统计学意义(P>0.05)。结论中药清毒栓可使HPV感染转阴,并可能通过上调阴道免疫微环境IFN-γ水平起到抗病毒的作用。Objective To study the efficacy of Qingdu suppository and interferon α-2b on cervical high-risk HPV infection as well as its regulation effects on vaginal immune microenvironment, so as to explore the possible immune regulation mechanism of Qingdu suppository. Results According to the treatment interest, patients who met diagnostic criteria of cervical high-risk HPV infection were divided into a Qingdu suppository group, a interferon α-2b group and a follow-up group. Three treatment sessions were given, and follow-up visit was performed in the first month after treatment. The vaginal washing fluid was collected before the treatment and during the follow-up. The ELISA method was used to measure the level of interferon-γ and interleukin-10 (IL-10). The clinical efficacy for high-risk HPV infection was analyzed to discuss the correlation among medication, efficacy and vaginal immune microenvironment. Results The negative conversion ratio was 38.46% in the Qingdu suppository group, 30. 00% in the interferon α-2b group and 20. 00% in the follow-up group, which was not significantly different ( P 〉 0. 05 ). After the treatment of Qingdu suppository and interferon α-2b, the lvel of interferon-γ was increased in the vaginal washing fluid, while the difference of IL-10 before and after treatment was not statistically significant (P 〉 0.05).
