基础胰岛素起始联合口服药治疗2型糖尿病112例临床分析

摘　　要：目的观察基础胰岛素起始联合口服药治疗2型糖尿病(T2DM)患者转归、有效性和安全性。方法选取T2DM患者112例,所有患者纳入研究时口服药血糖控制不达标,起始采用基础胰岛素,并根据血糖监测结果调整胰岛素用量,口服药维持原剂量,治疗24w,观察患者治疗前后空腹血糖(FBG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbAlc)、体质量指数(BMI)等变化及低血糖发生情况。结果治疗24w后,血糖控制达标率72%,FBG、2hPG和HbAlc水平较前明显下降,差异有统计学意义(P<0.01);BMI水平较前略有下降,但差异无统计学意义(P>0.05)。治疗期间11例受试者发生低血糖事件12次,均为一般性低血糖。结论基础胰岛素起始联合口服药治疗T2DM达标率高,治疗效果确切,低血糖发生率低,且病人依从性良好,是门诊治疗的首选方案。Objective To observe the prognosis,efficacy and safety of basal insulin therapy combined with oral drugs for type 2diabetes mellitus（T2DM）.Methods All 112 patients with T2 DM who were brought into the experimental group were not treated effectively enough with oral drugs.Then we treated them initially with basal insulin.All the patients chose insulin glargine or insulin detemir according to their own preferences and we adjusted the dose of insulin without change of the oral drug to treat them for 24 weeks depending on their blood glucose levels.We have observed the changes of fasting blood glucose（FBG）,2hour postprandial blood glucose（2hPG）,glycosylated hemoglobin A1C（HbA1C）,body mass index（BMI）and the incidence of hypoglycemia before and after treatment.Results After having been treated by insulin glargine or insulin detemir for 24 weeks,the compliance rate of the treatment FBG、2hPG and HbAlc levels were decreased significantly and the difference was statistically significant（P〈0.01）.BMI levels were decreased slightly and the difference was not statistically significant（P〉0.05）.11 patients occurred hypoglycemia events 12 times during treatment and they were all general hypoglycemia.Conclusion Initial therapy with basal insulin for type 2 diabetes mellitus has high compliance rate,high effectiveness,low incidence of hypoglycemia and good patient compliance.The treatment is simple,convenient,effective and safe.It is a preferred treatment especially for the outpatients.
