摘　　要：目的 探讨噬血细胞性淋巴组织细胞增生症（HLH）肝功能损害的临床特点.方法 对2007年7月～ 2013年6月在我院确诊为HLH的99例患者的临床资料进行回顾性分析.结果 99例HLH患者中出现肝功能异常（肝酶和/或胆红素异常）者95例,以轻中度损害为主.肝功能损害主要表现为白蛋白（96.97％）、乳酸脱氢酶（92.93％）、γ-谷氨酰基转移酶（90.91％）、谷丙转氨酶（89.90％）、谷草转氨酶（86.87％）的异常改变.其中谷草转氨酶、白蛋白、凝血酶原时间及纤维蛋白原在存活组及死亡组之间比较差异有统计学意义（P＜0.05）,白蛋白是HLH患者预后不良的独立危险因素.将HLH患者肝功能进行Child-Pugh分级,不同分级组间患者的生存率比较差异有统计学意义（P＜0.05）.结论 HLH患者普遍存在不同程度的肝功能异常,对于临床上出现不明原因发热、脾大、血细胞减少的患者,在除外嗜肝病毒感染的情况下,如伴发肝功能异常,应警惕HLH.肝功能损害不仅对HLH的早期诊断有提示作用,且与患者预后密切相关.Objective To explore the clinical characters of hemophagocytic lymphohistiocytosis （HLH） complicated by hepatic dysfunction. Methods To analyze the data of 99 patients with hemoph- agoeytic lymphohistioeytosis admitted to our hospital from July 2007 to June 2013 retrospectively. Results Liver injmT（ abnormal liver enzyme or/and bilirubin） was found in 95 cases of 99 HLH patients. Most of the patients had mild to moderate liver damage, which mainly showed abnormal changes of albumin （96.97%）, lactic dehydrogenase （ 92. 93% ）, gamma-glutamyltransferase （ 90. 91% ）, glutamic-pyruvic transaminase （89.90%） , glutamic oxalaeetic transaminase（ 86.87% ）. There were statistical differences in glutamic oxa]aeetic transaminase, albumin, prothrombin tittle and fibrinogen between survival group and dead group （ P 〈 0.05 ）. To further analysis, albumin was an independent predictor for the poor prognosis of HLH patients. Conclusion The majority of patients with HLH have mild to moderate liver damage. If hepalitis virus infection is excluded, HLH should be considered when a patient with unexplained fever, splenomegaly and hypocytosis appears abnormal liver function. Hepatic dysfunction is not only helpful for the early diagnosis,but also closely related with the prognosis of HLH.
