摘 要:目的:评价X线平片、CT和MRI诊断腰椎结核的效果。方法:回顾性分析40例腰椎结核和52例其它腰椎疾病患者的影像学资料,比较其常用的诊断指标。结果:在诊断腰椎结核的正确率方面,MRI(94.6%)最高,CT(82.6%)次之,X线平片(71.7%)最低,差异均有统计学意义(P<0.05)。在诊断腰椎结核的灵敏度方面,MRI(95.0%)高于X线平片(62.5%)和CT(77.5%),差异均有统计学意义(P<0.05);但X线平片与CT之间的差异无统计学意义(P>0.05)。在诊断腰椎结核的特异度方面,MRI(94.2%)高于X线平片(78.8%),差异有统计学意义(P<0.05);但MRI与CT(86.5%)及CT与X线平片之间的差异无统计学意义(P>0.05)。结论:X线平片、CT和MRI在诊断腰椎结核方面各具优势,实际工作中应充分利用这三种影像学检查方法的优势,以X线平片检查为基础,辅以CT和MRI检查,为更准确地诊断腰椎结核提供影像学依据。Objective: To evaluate the result of X-ray, CT and MRI in the diagnosis of tuberculosis of lumbar spine. Methods: A retrospective analysis of 40 cases of tuberculosis of lumbar spine and 52 cases of other diseases of lumbar spine was conducted, and the common diagnostic indexes were then compared. Results: In the diagnostic accuracyof tuberculosis of lumbar spine, MRI(94.6%) was highest, CT(82.6%) secondary, X-ray(71.7%) lowest, all the differences were significant(P〈0.05). In the diagnostic sensitivity, MRI(95.0%) is higher than X-ray(62.5%)and CT(77.5%), the differences were significant(P〈0.05); But the difference between X-ray and CTwas not significant(P〉0.05). In the diagnostic specificity, MRI(94.2%) is higher than X-ray(78.8%), the difference was significant(P〈0.05); But the differences between MRI and CT(86.5%), and between X-ray and CT were not significant(P〉0.05). Conclusions: X-ray, CT and MRI had respective advantage in the diagnosis of tuberculosis of lumbar spine, CTand MRI should beappropriatelyused on thebasisofX-rayin clinicalwork.
