云南不明原因猝死病例报告质量分析

摘　　要：目的分析总结云南不明原因猝死病例报告的时限性、准确性,为今后制定病例报告措施提供科学依据。方法1978—2013年的病例报告分为2个年段,特别对2002年业务接管后的病例报告接收情况进行更详实的统计分析。结果 1978—2013年累计发生猝死事件181起,死亡442例;1978—2001年发生93起（51.4%）,猝死252例（57.0%）;2002—2013年发病88起（48.6%）,猝死190例（43.0%）,其中上报事件77起（87.5%）,猝死165例（86.8%）,2002—2013年上报的77起中符合云南不明原因猝死事件56起（72.7%）,猝死病例160例（84.2%）;经追溯有11起（12.5%）、25例（13.2%）未报;猝死事件中有20起（26.0%）在首发病例猝死后8小时内上报,5起（6.5%）在8~16小时内上报,10起（13.0%）在16~24小时上报,42起（54.5%）在2天后上报。结论随着人们对本病的认知,病情上报意识逐渐增强,上报率、及时性和准确性也在不断提高,为聚集性事件中续发病例救治、降低死亡率,以及在猝死事件的回顾性调查、有效标本采集等赢得了时间,从而使防控工作进一步开展得到更切实、有效的保障。Objective To analyze and summarize the timeliness and veracity of case reports on Yunnan sudden unexplained death（YSUD）, so as to provide scientific basis for formulating measures of future case reports. Methods The cases reports from 1978 to 2013 were divided into two groups of year, especially, the reception condition of the cases reports after the business was taken over in 2002, was made a more detailed analysis. Results From 1978 to 2013, there were 181 YSUD incidents cumulatively with the total deaths of 442 cases, and 93（51.4%） incidents with the total deaths of 252 cases（57.0%） from 1978 to 2001. Eighty-eight cases（48.6%） occurred with sudden death of 190 cases（43.0%）, including reported77 incidents（87.5%） with the total deaths of 165 cases（86.8%）. Out of the 77 reported incidents, 56（72.7%） accorded with the definition of YSUD, with 160 death cases（84.2%）. Eleven incidents（12.5%） with 25 cases（13.2%） failed to be reported.By analyzing the timelines of the reports, 20 incidents（26.0%） were reported within 8 hours after the initial case’s death, 5 incidents（6.5%） within 8 to 16 hours, 10（13.0%） within 16 to 24 hours, 42 incidents（54.5%） after 2 days. Conclusions With people cognizing the disease, their report awareness is gradually enhancing during managing the reporting work and the reported rate, timeliness, veracity also improve continually, which help saving much time for treating and curing the secondary cases, reducing the death rate and conducting retrospective investigation, collecting effective specimen, thus the further development of prevention and control work get more practical and effective protection.
