疏肝解郁汤治疗原发性胆汁返流性胃炎并焦虑抑郁的疗效观察

摘　　要：目的:探讨疏肝解郁汤治疗原发性胆汁返流性胃炎(PBRG)并焦虑抑郁患者的疗效。方法:100例合并抑郁焦虑状态的PBRG患者随机分成对照组和观察组各50例。对照组给予口服铝碳酸镁片和多潘立酮片治疗,观察组应用自拟疏肝解郁汤加减治疗。疗程均为4周。两组治疗前后分别进行症状评分、胃镜评分、汉密尔顿抑郁量表(HAMD)与汉密尔顿焦虑量表(HAMA)评分和疗效评定。并观察药品不良反应发生情况。结果:治疗后,对照组总有效率为68.0%。观察组总有效率为96.0%。两组疗效差异有统计学意义(P<0.05)。两组治疗后症状积分和胃镜评分均有明显下降(P<0.05),且观察组较对照组降低更明显(P<0.05)。治疗后,观察组HAMD和HAMA评分较前明显降低(P<0.05),且明显低于对照组(P<0.05),而对照组治疗前后HAMD和HAMA评分比较,差异无统计学意义(P>0.05)。结论:采用疏肝解郁汤治疗原发性胆汁返流性胃炎并焦虑抑郁效果显著。Objective：To investigate the effect of Shugan Jieyu Decoction on the treatment of primary biliary re？ux gastritis（PBRG）with anxiety and depression. Methods：100 patients with depression were randomly assigned to control group and observation group each 50 cases. The control group were treated with hydrotalcite and domperidone,and the observation group with Shugan Jieyu decoction. Their symptoms,gastroscope scale,Hamilton depression scale,hamilton anxiety scale and therapeutic effect were evaluated. Results：After the treatment,the total effective rate was 68% in the control group and the total effective rate was 96% in the observation group. There was significant difference in the two groups（P〈 0. 05）. After the treatment,the symptom score and gastroscope scale obviously descended in the two groups,but they declined more obviously in the observation goup than in the control group. The Hamilton depression scale and Hamilton anxiety scale declined more obviously in the observation group than in the control group（ P〈 0. 05）. In the HAMA /HAMD,there was obvious difference in the observation group（ P〈 0. 05）,but no difference in the control group. Conclusion：Shugan Jieyu decoction could bring about its remarkable effect on the treatment of primary biliary reflux gastritis with anxiety and depression.
