中青年股骨颈骨折中空钉内固定术联合带髋周肌骨瓣植骨术疗效分析

摘　　要：目的:探讨中青年股骨颈骨折中空钉内固定手术联合带髋周肌骨瓣植骨手术的临床疗效。方法:回顾性分析本院2005年12月-2010年12月收治的62例中青年股骨劲骨折患者的临床资料、手术方法及随访结果。结果:术后3、6、9、12、24、36个月时间,对患者进行进行X线片检查,骨折愈合患者56例,占90.3%;4例患者因愈合延迟导致退钉,更换螺钉后骨折愈合;2例患者术后6个月,出现股骨头的坏死,后采用全髋关节置换术治疗;术后采用Harris髋关节功能评分,优:56例,占90.3%,良4例,占6.5%,差2例,占3.2%,治疗的优良率为96.8%。结论:中青年股骨颈骨折患者采用中空钉内固定手术联合带髋周肌骨瓣植骨手术,股骨颈正常解剖关系得到恢复,骨折固定稳固,明显改善股骨颈血液供应,具有良好的骨折愈合率。同时,可以避免或延缓人工全髋关节置换术,可很好恢复患者肢体功能,提高了生活质量,值得推广应用。Objective：To explore the effect of hollow screw fixation combined with hip weeks musculoskeletal flap graft surgery in treatment of young and middle-aged femoral neck fracture.Method：The clinical data,surgical techniques and follow-up results of 62 patients with young and middle-aged femoral neck fracture who were treated in our hospital were retrospective analyzed.Result：The patients were detected by X-ray after 3,6,9,12,24 and 36 months of surgery.And the number of fracture healing was 56 cases（90.3%）;there were 4 cases to retreat nails because of healing delay,and after replacing nails,the 4 cases became fracture healing;2 cases happened osteonecrosis after 6 months of surgery,and then they were treated by total hip arthroplasty.According to the Harris hip score after surgery,the number of excellent patients were 56 cases（90.3%）,the number of good patients were 4 cases（6.5%）,the number of bad patients were 2 cases（3.2%）,the good and excellent rate was 96.8%.Conclusion：Hollow screw fixation combined with hip weeks musculoskeletal flap graft surgery in treatment of young and middle-aged femoral neck fracture can recover the normal anatomy of the femoral neck,fix the fracture stably,improve the blood supply to the femoral neck,and have a good fracture healing rate.It also can avoid and delay total hip arthroplasty,and have a good effect in body function recover and improve the quality of life,so it is worthy of clinical promotion.
