耳穴电针治疗心脾两虚型抑郁症患者23例随机单盲试验

摘　　要：目的评价电针耳穴"心"、"脾"治疗心脾两虚型抑郁症临床疗效及安全性。方法 48例轻、中度抑郁症患者随机分为治疗组(23例)及对照组(21例),治疗组采用华佗牌耳夹刺激仪刺激耳穴"心"、"脾",对照组采用假耳夹在后端输出电流,刺激耳穴"肩"、"肘"。选用连续波,输出脉冲频率20Hz,强度以忍受而不产生疼痛为度。每次治疗20 min,每日2次,每周连续治疗5天,共治疗4周。比较治疗前后患者汉密尔顿抑郁量表24项(HAMD-24)、抑郁自评量表(SDS)、中医证候量表评分,并评价临床疗效,记录不良反应。结果治疗组临床疗效总有效率为60.87%,对照组为10.53%,治疗组优于对照组(P<0.01)。两组治疗后HAMD-24评分、SDS评分、中医证候评分均较本组治疗前显著降低(P<0.01),并且治疗组各项评分较对照组降低明显(P<0.01)。两组均未见明显不良反应。结论电针耳穴"心"、"脾"能改善抑郁症患者临床症状,其临床疗效安全有效。Objective To evaluate the efficacy and safety of auricular electro-acupuncture （EA） on auricular points ‘heart＇ （CO15） and ‘spleen＇ （CO13） for depression with heart-spleen deficiency syndrome. Methods Totally 48 depression patients with heart-spleen deficiency syndrome were randomized into the treatment group of 23 cases and control group of 21 cases. The treatment group was given auricular EA on CO15 and CO13 for 20 minutes twice daily. The control group was given auricular EA on ear points of ‘shoulder＇ and ‘ elbow＇ for 20 minutes twice daily. The patients were treated 5 days weekly and the treatment lasted for 4 weeks. The scores of Hamilton Rating Scale for Depression-24 edition （24HAMD）, Self-rating Depression Scale （SDS） and Traditional Chinese Medicine （TCM） Syndrome Scale were compared. The efficacy was evaluated. The adverse reactions were recorded. Results The total effective rate of the treatment group and control group was 60. 87% and 10.53%, respectively, with a significant difference between groups （P 〈 0.01 ）. The scores of 24HAMD, SDS and TCM Syndrome Scale were sig- nificantly decreased in both groups after treatment （P 〈0.01 ） , especially in the treatment group （P 〈0. 01 ）. There was no adverse reaction in both groups. Conclusion Auricular EA on CO15 and CO13 is effective and safe for de- pression. It can obviously relieve the symptoms of patients with depression.
