摘  要:目的:了解综合干预措施对耐甲氧西林金黄色葡萄球菌(MRSA)医院感染预防与控制的效果。方法:采取回顾性与前瞻性调查方法分别对未干预组和干预组MRSA医院感染病例的预防和控制效果进行调查分析。结果:MRSA医院感染总发生率为0.15%,病死率为9.40%。感染部位以下呼吸道感染为主(87.71%),手术伤口感染居第2位(5.93%)。MRSA对11种抗菌药物的耐药率大于80%。MRSA医院感染发生率和病死率(0.24%、11.11%)在综合干预后明显下降(0.11%、7.69%)。手术伤口感染率在干预后下降明显,由8.40%降至3.24%。不同临床科室中,神经内科和综合ICU取得较为显著的控制效果,感染所占比例分别由干预前的21.85%、14.29%降至干预后的8.55%、7.69%。结论:综合干预措施能有效预防和控制MRSA医院感染。OBJECTIVE: To understand the prevention and control effects of comprehensive intervention measure on nosocomial infection induced by methicillin-resistant Staphylococcus aureus (MRSA). METHODS: Retrospective and prospective investigation method were adopted to analyze the effects of prevention and control for nosocomial infection induced by MRSA between non-intervention group and intervention group. RESULTS: The total incidence of nosocomial infection induced by MRSA was 0.15%, and the mortality was 9.40%. The lower respiratory tract infection was the primary among infection sites (87.71% ), followed by operation wound infection (5.93%). Drug resistance of MRSA to 11 kinds of antibacterials was more than 80%. After comprehensive intervention, the incidence and mortality of nosocomial infection induced by MRSA decreased significantly from 0.24% and 1 1.1 1% to 0.11% and 7.69%, respectively. The incidence of operation wound infection decreased significantly from 8.40% to 3.24% after intervention. Among different departments, nosocomial infection control obtained significant effects in neurol- ogy department and integrated ICU. The proportion of nosocomial infection in neurology department and integrated ICU decreased from 21.85% and 14.29% before intervention to 8.55% and 7.69% after the intervention. CONCLUSIONS: Comprehensive intervention measures can effectively prevent and control nosocomial infection induced by MRSA.
