内痔注射横扎并整形小切口外痔切除术治疗环状混合痔98例疗效观察

摘　　要：目的观察内痔注射横扎并整形小切口外痔切除术治疗环状混合痔的临床疗效。方法将194例环状混合痔患者随机分为2组,治疗组98例予内痔注射横扎并整形小切口外痔切除术,对照组96例予外剥内扎硬注术。观察2组疗效、手术后视觉模拟评分法(VAS)评分变化;观察手术后水肿、出血、肛门失禁情况;比较手术时间、创面愈合时间;观察术后并发症发生情况,包括尿潴留、肛门狭窄;观察术后6个月复发情况。结果 2组治愈率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组手术后VAS评分比较差异有统计学意义(P<0.05),治疗组手术后疼痛较对照组减轻。2组水肿、出血、肛门失禁情况比较差异均有统计学意义(P<0.05),治疗组手术后水肿、出血、肛门失禁情况均较对照组减轻。治疗组手术时间较对照组短(P<0.05),创面愈合时间较对照组提前(P<0.05)。治疗组手术后尿潴留、肛门狭窄发生率低于对照组(P<0.05)。结论内痔注射横扎并整形小切口外痔切除术治疗环状混合痔,疗效确切,简便易行,费用低廉,适合在基层医院推广。Objective To observe the clinical effect of circular mixed hemorrhoid treated by internal hemor -rhoid injection transverse combined with plastic small incision resection .Methods 194 cases of circumferential mixed hemorrhoid were chosen and randomly divided into treatment group （98cases）, control group （96 cases）. Hemorrhoids in treatment group used by cross and plastic resection of small incision external hemorrhoid cure , cases in control group adopts outer peel inside hard injection technique .The clinical curative effect , operation time and wound healing time were observed;the postoperative pain, edema, bleeding, urinary retention, anal stenosis, anal in-continence and other major complications were observed;The postoperative recurrence of 6 months were observed . Results The clinical efficacy of treatment group is better than that of control group （P〈0.05）;Main complications after treatment in treatment group were lower than control group （P〈0.05）;Operation time and wound healing time is shorter than the control group （P〈0.05）;6 months postoperative recurrence rate in treatment group is lower than the control group （P〈0.05）.Conclusion Internal hemorrhoid injection transverse combined with plastic small in-cision resection in the treatment of circumferential mixed hemorrhoid is exact , simple and low cost of operation meth-od, suitable for promotion in grassroots hospital .
