摘 要:目的探讨大叶性肺炎患儿血清TNF-α、IL-6、IL-8、IL-10和高迁移率族蛋白BI(HMGBl)水平的变化及其相关性。方法采用双抗体夹心酶联免疫吸附法(ELISA)检测10例健康对照儿童、3l例轻度大叶性肺炎患儿和14例重度大叶性肺炎患儿的血清TNF-α、IL-6、IL-8、IL-10和HMGBI水平,分析不同程度肺炎患儿血清HMGBl、TNF-α、IL-6、IL-8、IL-10水平的差异,及血清中TNF-α、IL-6、IL-8、IL-10与HMGBl水平的相关性。结果重度大叶性肺炎组患儿、轻度大叶性肺炎组患儿血清TNF-α、IL-6、IL-8、IL-10和HMGBl水平均显著高于健康对照组(P均〈0.05);重度大叶性肺炎组患儿和轻度大叶性肺炎患儿血清TNF-α、IL-6、IL-8、IL-10和HMGBl水平比较差异均有统计学意义(P均〈0.05)。TNF-α、IL-6、IL-8与HMGBl呈中度正相关关系(r=0.444、0.365、0.466,P=0.007、0.029、0.004);IL-10与HMGBl呈中度负相关(r=-0.485,P=0.003)。结论大叶性肺炎患儿血清TNF-α、IL-6、IL-8、IL-10、HMGBl与疾病严重程度相关,而TNF-α、IL-6、IL-8、IL-10、HMGBl水平测定可为小儿大叶性肺炎病情进展监测、严重程度及疗效判定等提供参考。Objective To investigate the concentration changes and the inter-relationship of tumor necrosis factor-m( TNF-α ), interleukin-6 ( IL-6 ), interleukin-8 ( IL-8 ), interleukin-10 ( IL-10 ) and high mobility group box-1 ( HMGBI ) in serum of children with lobar pneumonia. Methods By double-antibody sandwich enzyme linked immu- nosorbent assay, the serum TNF-α, IL-6, IL-8, IL-1O, HMGB1 levels were determined in 10 healthy children, 31 chil- dren with mild lobar pneumonia and 14 children with severe lobar pneumonia. The correlations between serum TNF-α, IL-6,IL-8 ,IL-10 with HMGBI were analyzed. Results The levels of serum TNF-α,IL-6 ,IL-8 ,IL-10 and HMGBI in children with severe lobar pneumonia and children with mild lobar pneumonia were significantly higher than those in healthy children ( all P 〈 O. 05 ). There were significant differences in TNF-α, IL-6, IL-8, IL-10 and HMGB1 between children with mild lobar pneumonia and children with severe lobar pneumonia ( all P 〈 0.05 ). There was relationship between TNF-α, IL-6, IL-8 and HMGB1, showing moderate positive correlation, and the correlation coefficients were re- spectively 0.444,0. 365 and 0. 466 ,P = 0. 007,0. 029,0. 004 ; IL-10 and HMGB1 had moderate negative correlation, the correlation coefficient was 0. 485,P = 0.003. Conclusions For children with lobar pneumonia, serum concentration of TNF-α,IL-6,IL-8, IL-10 and HMGB1 were correlated with infectious status. To detect the serum level of TNF-α, IL-6, IL-8, IL-IO and HMGBI ,can provide some reference for clinical practice.
