普胸术后出血再次剖胸止血的分析

摘　　要：目的：探讨普胸术后胸内活动性出血的原因，预防及治疗指征。方法选取我院2012年4月至2014年4月普胸术后大出血再次开胸止血治疗的43例患者的出血原因、部位及诊治经过进行回顾性分析。结果所有患者均痊愈出院。无死亡及胸腔感染患者，其中有1例经再次剖胸止血后胸腔引流液仍然较多，经复查床旁胸片显示胸腔内血块形成而第三次开胸止血均为胸壁粘连电凝止血处渗血，止血胶＋创面予电凝成功止血。结论普胸手术要预防活动性出血的发生。术中必须仔细观察、认真操作。术后若发生活动性出血要积极再次剖胸止血。ObjectiveTo investigate the causes of postoperative intrathoracic＇s chest active bleeding and processing method. Methods The clinical data of 43 patients of postoperative thoracic hemorrhage reason given treatment of hemostasis again were selected from April 2012 to April 2014 in our hospital. Their data of hemorrhage causes, parts and the course of diagnosis and treatment were retrospectively analyzed.Results All patients were recovered with no death and chest infection patients. But in 1 case chest drainage fl uid was still more after hemostasis by cutting the chest again, he was reviewed by bedside chest X-ray and it showed intrathoracic blood clot formation, and the third open thoracic bleeding was done by chest wall adhesion in an electrocautery hemostasis in which hemostatic gel with the electric coagulation to the wound were used to obtain successful stop bleeding.Conclusion We need to prevent the happening of the active bleeding in general thoracic surgery. Carefully watching and seriously operating should be performed in the intraoperative operation. In case of postoperative active bleeding, intrathoracic chest hemostasis should be given to stop the bleeding.
